إن التمييز بين أمراض مفصل الكتف تعتبر في منتهى الصعوبة و لهذا فإن الفحص بالتصوير المسحي يلعب دورا هاما في تقييم هذه الحالات.
و تعد الموجات فوق الصوتية آمنة، سريعة، بدون إشعاع بالإضافة إلى إمكانية التصوير أثناء الحركة.
و قد تم إجراء هذا البحث لتقييم مدى فائدة الفحص بالموجات فوق الصوتية لتشخيص أية تغييرات في الأنسجة المحيطة بمفصل الكتف.
و لهذا تم اختيار 40 مفصل للكتف بها الآم قيموا إكلينيكيا و تم تشخيصهم بواسطة الموجات فوق الصوتية ثم تم تعزيز التشخيص بالرنين المغناطيسي.
و أضحت إن الموجات فوق الصوتية موثوق بها لتشخيص حالات القطع الكلي للأوتار و التهاب الأوتار التكلسي و كذلك انحلال مفصل acromio-clavicular (حساسية 100 %، دقة 100 %).
بينما أخفقت الموجات فوق الصوتية في تقييم بعض حالات القطع الجزئي للأوتار (دقة 92.5 %).
و التهاب الأوتار المصاحب الكيسي (دقة 92.5 %).
و أيضا حالات الاحتباس في مفصل acromio-clavicular (دقة 95 %).
و قد خلصت الدراسة إلى أن الموجات فوق الصوتية دقيقة للغاية لتشخيص حالات اضطراب الأنسجة الرخوية، و بذلك تكون امتداد طبيعي للفحص الإكلينيكي الخاص بالجهاز العضلي، كما أنها اقتصادية و سريعة بالمقارنة بالرنين المغناطيسي.
Abstract EN
Hypothesis: Distingiushing between shoulder disorders can sometimes be difficult, where an imaging technique is needed for further evaluation of such cases.
Ultrasound (US) is a fast, safe, inexpensive, widely available, totally non-invasive, radiation free and can provide a dynamic view of moving parts.
Materials and Methods: This present study was performed to assess the usefulness of ultrasound examination in detecting the pathology of the painful shoulder.
Forty painful shoulders were assessed clinically and diagnosed with US examination.
The diagnosis was confirmed with magnetic resonance imaging (MRI).
Results: US was found to be reliable and highly accurate for the detection of full-thickness tears, calcific tendinitis and acromioclavicular joint (ACJ) degeneration (accuracy = 100%).
In the assessment of partial thickness tear, rotator tendinitis with bursitis and ACJ impingement, US failed to diagnose some cases (accuracy = 92.5%, 92.5% and 95% respectively).
Conclusion: US is an accurate, highly specific tool in the diagnosis of soft tissue disorders, being a logical extension of the clinical examination of the musculoskeletal system.
Also, it is an economic, fast technique rather than the expensive MRI.
