إن العوامل المؤثرة في موت الخلية المبرمج تتفاعل في الغشاء الزلالي للمفاصل في مرضى الرثيان المفصلي.
أثبتت هذه الدراسة أن نسبة ظهور CD95 و BcI-2 لدى الخلايا الليمفاوية في الدم لا تختلف بطريقة ذو دلالة إحصائية عند مقارنة 20 مريضا بمرض الرثيان المفصلي و المجموعة الضابطة بينما وجد اختلاف ذو دلالة إحصائية في كل من عامل التنكرز للأورام –ألفا و جزئ ما بين الخلايا اللاصق -1 الذائب عند مقارنة معدلاتها في مصل مجموعة المرضى و مجموعة الأصحاء.
و قد أوضحت هذه الدراسة وجود اختلاف ذو دلالة إحصائية عالية بين هذه المؤشرات (CD95 و BcI-2 )، عامل التنكرز للأورام-ألفا و جزئ ما بين الخلايا اللصق -1 الذائب) عند مقارنة معدلاتها في المصل و السائل الزلالي لمجموعة المرضى.
و أظهرت النتائج أن هناك علاقة بين درجة نشاط المرض و شدته و مستوى هذه المؤشرات خاصا في السائل الزلالي.
نستنتج من هذا البحث أن هناك عدم توازن بين العوامل المتسببة في موت الخلية المبرمج(CD95) و العوامل المضادة له (BcI-2) كما أن زيادة كل من عامل التنكرز للأورام-ألفا و جزئ ما بين الخلايا اللصق-1 الذائب تعتبر من العوامل التي تنظم عملية موت الخلية المبرمج.
إن وضوح دور العوامل المؤثرة في موت الخلية المبرمج قد يكون له أثر هام في التدخل العلاجي لحالات مرض الرثيان المفصلي.
Abstract EN
In this study, the percentage of CD95+ and Bcl-2+ peripheral blood lymphocytes of 20 active rheumatoid arthritis (RA) patients showed no significant difference as compared to healthy controls.
Tumor necrosis factor-alpha (TNF-?) and soluble intercellular adhesion molecule-1 (sICAM-1) were significantly higher in the sera of RA patients compared to the control subjects.
Highly significant differences were observed in the results of CD95%, Bcl-2%, TNF-? and sICAM-1 when synovial fluid samples were compared with the peripheral blood of RA patients.
A significant association between these parameters and both disease activity and severity were also demonstrated.
These results indicate an imbalance between the apoptotic factors that are represented in this study by CD95 and the anti-apoptotic factors represented by Bcl-2.
The increased TNF-? and sICAM-1 are two important factors regulating apoptosis.
The elucidation of the biochemical pathways and specific proteins that regulate the interaction between apoptotic and anti-apoptotic factors will provide a remarkable opportunity to manipulate the life-and-death cell decisions through novel therapeutic interventions.
