الهدف من هذا البحث هو مقارنة الخصائص الإكلينيكية و التصوير بالأشعة السينية و النظائر المشعة في تقييم التهاب المفاصل العظمى لليدين.
اشتمل البحث على 30 مريضا مصابين بالتهاب المفاصل العظمي لليدين و 10 أشخاص أصحاء كمجموعة ضابطة و قد أجري لهم الآتي :- 1-فحص إكلينيكي شامل مع التركيز على مفاصل اليدين و الرسغين.
2-الاختبارات المعملية آلاتية :- أ- صورة دم كاملة.
ب-سرعة ترسيب كرات الدم الحمراء.
ج-اختبار لاتكس للمعامل الرثياني.
د-مستوى حامض البوليك في الدم.
3-التصوير بالأشعة السينية لليدين و الرسغين.
4-التصوير بالنظائر المشعة لليدين و الرسغين.
بمقارنة الخصائص الإكلينيكية و التصوير بالأشعة السينية و النظائر المشعة في تقييم التهاب المفاصل العظمى لليدين وجد الآتي :- 1-أكثر المفاصل إصابة بالتهاب المفاصل العظمي باليدين هي المفاصل الطرفية للأصابع و المفصل الأول لإصبع الإبهام و ذلك باستخدام كل من التصوير بالأشعة السينية و النظائر المشعة.
2-وجدت علاقة ذات قيمة إحصائية بين درجة الإحساس بالألم و كلا من الزوائد العظمية و تصلب ما تحت الغضاريف بينما لا توجد علاقة ذات دلالة إحصائية بين درجة الإحساس بالألم و كلا من الحويصلات العظمية و ضيق الفراغ بالمفصل.
3-لا توجد علاقة ذات دلالة إحصائية بين درجة الضجر و نتائج الأشعة السينية.
4-توجد علاقة ذات دلالة إحصائية بين شدة الإصابة بالمرض عند الفحص بالنظائر المشعة و درجة الإحساس بالألم و درجة الألم بالضغط.
5-هناك علاقة ذات دلالة إحصائية بين شدة الإصابة بالمرض عند الفحص بالنظائر المشعة و بين الزوائد العظمية و تصلب ما تحت الغضاريف و لكن لا توجد علاقة ذات قيمة إحصائية بين شدة الإصابة بالمرض عند الفحص بالنظائر المشعة و بين الحويصلات العظمية و ضيق الفراغ بالمفصل.
نستنتج من هذا البحث أن التصوير بالنظائر المشعة سيكون له دور هام في تقييم مرض التهاب المفاصل العظمي لليدين.
Abstract EN
Objective: To compare the clinical features, macro-radiography and scintigraphy in the assessment of osteoarthritis (OA) of the hands.
Materials & Methods: Thirty patients with OA of the hands and 10 healthy controls were included in this study.
They were subjected to thorough clinical examination, laboratory investigations (full blood picture, ESR, rheumatoid factor and serum uric acid), macroradiography and radioisotope scanning of both hands and wrists.
Results: Both methods of joint imaging showed the predominant involvement of the distal interphalangeal (DIP) and first carpometacarpal (1st CMC) joints.
There was a significant correlation between pain score and both osteophytes and subchondral sclerosis while there was a non-significant correlation between pain score and both joint space narrowing and cysts.
There was a non- significant correlation between the articular index and all radiographic data.
Also, there was a significant correlation between bone scan score and pain score, articular index, osteophytes and subchondral sclerosis.
There was a non-significant correlation between bone scan score and both joint space narrowing and cysts.
Conclusion: Scintigraphy could be considered a sensitive technique for monitoring osteoarthritis of the hands.
