أجريت هذه الدراسة على ثلاثين مريضا بالرثيان المفصلي (22 أنثى، ٨ ذكور) و كان متوسط أعمارهم 42.6 ± 8.3 سنة.
و قد تم أخذ التاريخ المرضي كاملا و تم لهم فحص إكلينيكي دقيق لجميع المرضى و كذلك تم عمل الفحوص المعملية و الأشعات و تم أيضا لجميع المرضى عمل أشعة مقطعية عالية الدقة بالحاسوب الآلي.
و أظهرت هذه الدراسة توسع الشعب الهوائية في 12 مريضا (40 %) و قد وجد أنه أكثر الأمراض المكتشفة شيوعا بالأشعة المقطعية عالية الدقة بالحاسوب الآلي و كان أكثر الأماكن شيوعا لتوسع الشعب الهوائية هي الفص السفلي الأيسر (58.33 %)، و الفص الأوسط (50 %)، و كان أكثر أنواع توسع الشعب الهوائية هو النوع الأسطواني (66.33 %).
و قد وجد أن معدل انتشار توسع الشعب الهوائية كان أعلى بدلالة إحصائية في مرضى الرثيان المفصلي الذكور.
(50 % في الذكور و 36.3 % في الإناث) و كان عمر المرض أقل بدلالة إحصائية و مدة مرض الرثيان المفصلي كانت أعلى بدلالة إحصائية في المرضى المصابون بتوسع الشعب الهوائية عنها في المرضى الغير مصابين بتوسع الشعب الهوائية.
و لم يوجد أي اختلاف ذو دلالة إحصائية بين مرضى الرثيان المفصلي المصابون بتوسع الشعب الهوائية و الغير مصابين بتوسع الشعب الهوائية بالنسبة لوجود ظواهر خارج المفاصل أو وجود تشوهات أو تآكلات عظمية أو معامل الروماتويد الإيجابي أو استخدام الكورتيزونات أو مثبطات المناعة و كذلك بالنسبة للتدخين أو وجود مرض رئوي سابق أو أعراض تنفسية أو اختلال في اختبارات وظائف الرئة و كذلك لوجود إصابات أخرى بالصدر في الأشعة المقطعية عالية الدقة بالحاسوب الآلي.
و يستخلص من هذا البحث أن توسع الشعب الهوائية يمكن اعتباره تغيرا رئويا مميزا في مرضى الرثيان المفصلي، و الاستخدام الروتيني للأشعة المقطعية عالية الدقة بالحاسوب الآلي للصدر قادر على توضيح توسع الشعب الهوائية (الغير مصحوب بأعراض) و الذي يعتبر أكثر تغير رئوي شيوعا في مرضى الرثيان المفصلي.
Abstract EN
Materials & Methods: The study was conducted on thirty rheumatoid arthritis (RA) patients.
They were 22 females and 8 males, with a mean age of 42.6±8.3 years.
All patients were subjected to complete history taking, thorough clinical examination and routine laboratory investigations.
High resolution computed tomography (HRCT) of the chest was performed for all patients as well.
Results: Bronchiectasis was found in 12 patients (40%) and was the most common chest abnormality detected with HRCT.
The most common sites of bronchiectasis were the left lower lobe (58.33 %) and the middle lobe (50%).
Bronchiectasis was mainly of the cylindrical type (66.33 %).
There was a significant higher prevalence of bronchiectasis in male patients (50%) than in female patients (36.3%).
The age of patients was significantly lower and the duration of RA was significantly higher in patients with bronchiectasis when compared with those without.
No significant differences were found between RA patients with and without bronchiectasis regarding extra-articular manifestations, presence of deformities, bony erosions, positive rheumatoid factor, use of corticosteroids or immuno suppressives, smoking history, presence of previous lung disease, respiratory symptoms, abnormal pulmonary function tests or presence of other chest lesions on HRCT.
Conclusion: Bronchiectasis might be a characteristic lung affection in RA patients and routine use of HRCT of the chest is recommended for demonstration of silent bronchiectasis.
