كان الهدف من هذا البحث هو دراسة الخصائص الإكلينيكية و التغيرات الإشعاعية في مرض الالتهاب المفصلي العظمي بالركبة و ارتباطها بوجود بلورات الكالسيوم بيروفوسفات داي هيدرات و الهيدروكسي أبيتات في السائل الزلالي.
اشتمل البحث على (60) مريضا بالالتهاب المفصلي العظمي بالركبة و قد أجري لهم فحص إكلينيكي شامل و أشعة إكس على الركبة (أمامي خلفي و جانبي) و الاختبارات المعملية الآتية : 1- صورة دم كاملة.
2- سرعة ترسيب كرات الدم الحمراء.
3- المعامل الرثياني في المصل.
4- تحليل السائل الزلالي للبحث عن عدد كرات الدم البيضاء و وجود بلورات الكالسيوم بيروفوسفات داي هيدرات و الهيدروكسي أبيتات.
و قد وجد من خلال هذا البحث أن 17 مريضا (28.3%) بمرض الالتهاب المفصلي العظمي بالركبة لديهم بلورات الكالسيوم بيروفوسفات داي هيدرات و 33 مريضا (55%) لديهم بلورات الهيدروكسي أبيتات و خمس من المرضى السبعة عشر (29.4 %) الذين لديهم بلورات الكالسيوم بيروفوسفات داي هيدرات مصابين أيضا بتعظم غضروف الركبة الذي تم تشخيصه من خلال أشعتهم.
و لقد وجد أن هناك علاقة ذات دلالة إحصائية بين وجود بلورات الكالسيوم بيروفوسفات داي هيدرات و التقدم في العمر في حين لا وجود لهذه العلاقة في حالة بلورات الهيدروكسي أبيتات.
كما وجدت هذه الدراسة علاقة ارتباط ذات دلالة إحصائية بين البلورات بنوعيها و شدة الألم و درجة التورم في مفصل الركبة.
و عامة فقد وجد هذا البحث أن البلورات بنوعيها ترتبط بعلاقة ذات دلالة إحصائية و درجة التغيرات الإشعاعية لمرض الالتهاب المفصلي العظمي بالركبة.
أما إذا أخذت هذه التغيرات كلا على حدة فسوف نجد أن بلورات الكالسيوم بيروفوسفات داي هيدرات ترتبط بعلاقة ذات دلالة إحصائية مع ضيق الفراغ بالمفصل و الزوائد العظمية و الحويصلات العظمية و تصلب ما تحت الغضاريف في صور الأشعة بينما لا توجد هذه العلاقة في حالة وجود بلورات الهيدروكسي أبيتات إلا مع ضيق الفراغ بالمفصل و الزائد العظمية.
و في نهاية هذه الدراسة نستطيع أن نؤكد على وجود ارتباط بين وجود بلورات الكالسيوم بيروفوسفات داي هيدرات و بلورات الهيدروكسي أبيتات و الالتهاب المفصلي العظمي بالركبة و أن التغيرات الإشعاعية الشديدة التي تظهر في حجيرات متعددة في المفصل تكون مصاحبة لوجود بلورات الكالسيوم بيروفوسفات داي هيدرات و الهيدروكسي أبيتات.
Abstract EN
Objective: To study the clinical and radiographic features of osteoarthritis (OA) of the knee associated with calcium pyrophosphate dihydrate (CPPD) and hydroxyapatite (HA) crystals deposition.
Materials & Methods: Sixty patients suffering of OA of the knee were included in this study.
They were subjected to thorough clinical examination, laboratory investigations: CBC, ESR, serum rheumatoid factor and synovial fluid analysis for the presence of CPPD and HA crystals.
They also had knee radiographs (antero-posterior standing & lateral 20* flexion).
Results: Seventeen patients (28.3%) had CPPD crystals and 33 patients (55%) had HA crystals in their synovial fluid.
Five out of 17 patients (29.4%) with CPPD crystals had chondrocalcinosis.
Patients with CPPD and HA crystals had significantly higher scores for pain, tenderness and swelling than those without.
Also, patients with CPPD and HA crystals had higher OA radiographic grading than those without.
Patients with CPPD crystals had higher scores for joint space narrowing, osteophytes, cysts and sclerosis while patients with HA crystals had higher scores for joint space narrowing and osteophytes.
Both CPPD and HA crystals were found in knees with bicompartmental and tricompartmental involvement.
Conclusion: The study confirms the association of CPPD and HA crystals with OA of the knee.
Severe and multi-compartmental radiographic changes are associated with CPPD and HA crystals deposition.
