Abstract AR
أجري هذا البحث بمستشفى أسيوط الجامعي على 100 مريض من مرضى العيادات الخارجية لقسم الروماتيزم و التأهيل.
و قد قسم المرضى إلى مجموعتين : المجموعة الأولى : عبارة عن 60 شخصا يعانون من الأعراض المرضية للركبة.
المجموعة الثانية : عبارة عن 40 شخصا لا يعانون من أي أعراض مرضية ظاهرية.
و لقد تم أخذ التاريخ المرضي و فحص إكلينيكي و اختبارات معملية عبارة عن (سرعة ترسيب سي رياكتف بروتين وعامل روماتويد و أخيرا الفحص بجهاز التصوير الحراري للمجموعتين من الأشخاص.
و لقد كان عدد المرضى الذكور في المجموعة الأولى 47 مريضا أما الإناث فكن 13 و كانت أعمارهم تتراوح ما بين 21 عاماً و 63 عاما و كانت مدة المعاناة من مرض الفصال العظمي المفصلي في الركبة تتراوح ما بين 3 سنوات إلى 12 سنة.
و من نتائج الفحص الاكلينيكي للمرضى الذين يعانون من التهاب الركبة اليمنى 10 مرضى و الذين يعانون من التهاب الركبة اليسرى 11 مريضا، بينما الذين يعانون من التهاب الركبتين كان عددهم 39 مريضا أيضا و بالفحص الاكلينيكي وجد أيضا أن 27 مريضا يعانون من دفئ ظاهري على الركبة و 57 مريضا يعانون من ألم أثناء الفحص.
و بينما لوحظ وجود نسبة تحديد في مدى الحركة في مفصل الركبة في 26 مريضا (في صورة عدم اكتمال مجال الحركة و انحناء خارجي في مفصل الركبة) بينما كان الارتشاح في مفصل الركبة موجود في 29 مريضا.
و كانت نتائج الفحص بجهاز التصوير الحراري الدالة على وجود التهاب بالمفصل تتمشى مع ما ذكر عاليا من نتائج الاكلينيكي.
أما النتائج المعملية، فقد أظهرت ارتباطا حسنا مع الفحص بجهاز التصوير الحراري أيضا و كما سبق نستخلص أن هناك ارتباطا وثيقا بين نتائج الفحص بجهاز التصوير الحراري و نتائج الفحص الاكلينيكي و المعملي التي تقاس في التهابات المفاصل و أن جهاز التصوير الحراري يساعد الطبيب في التكيف مع تشخيص التهاب المفاصل و يعتبر ذو حساسية و ليس له تأثير ضار أو جانبي و يمكن استخدامه بسهولة و في متابعة المرض.
Abstract EN
Patients & Methods: The study was conducted on 2 groups: symptomatizing group (60 patients with knee osteoarthritis) and a control group (40 normal persons).
Both groups were subjected to history taking, clinical examination, laboratory investigations and thermographic examination.
Results: In the first group, the number of males was (47) and females was (13) with age that ranged from 21 to 63 years.
The period of the disease ranged from 3 to 12 years.
Osteoarthritis of the right knee joint was present in 10 patients, left knee joint in 11 patients and both knees in 39 patients.
By clinical examination warmth was noticed in 27 patients, tenderness in 57 patients and deformity in 26 patients.
Effusion was detected in 29 patients.
Thermographic data indicating joint inflammation correlated with clinical examination data.
There was a strong correlation between thermographic findings and the laboratory parameters measured for joint inflammation.
Cocclusion: Thermography can help in establishing the diagnosis of joint inflammation.
As it is a sensitive non-specific and also non-invasive instrument, it can be used easily for diagnosis and for follow up of knee osteoarthritis
