يعد المفصل الحلقي الطهر جالي من المفاصل التي يمكن إصابتها بمرض الرثيان المفصلي و في هذا البحث بدراسة تأثر هذا المفصل بالمرض لدى ثماني و أربعين مريضا بالرثيان المفصلي و قد تم عمل الفحوصات الآتية : التاريخ المرضي بحثا عن أعراض إصابة الحنجرة.
الفحص الإكلينيكي لتقييم نشاط المرض و تحديد درجته من الناحية الوظيفة و كذلك وجود أعراض غير مفصلية و الأشعة السينية لليدين و القدمين.
فحص تام للأنف و الأذن و الحنجرة شاملا الفحص الحنجري غير المباشر بالمرآة.
اختبارات معملية (شاملة : الهيموجلوبين بالدم- سرعة الترسيب بعد الساعة الأولى و الثانية-عامل الروماتويد-البروتين المتفاعل "سي") التصوير الحنجري مع الاستروبوسكوب بالمنظار الأنفي البلعومي الليفي.
الرنين المغناطيسي للحنجرة و على الأخص : المفصل الحلقي الطهر جالي و لقد كانت الإصابة الحنجرية الناتجة عن كل من التصوير الأنفي الحنجري و الرنين المغناطيسي في 33 % من الحالات.
و تم مناقشة مشاهدات كل من التصوير الأنفي الحنجري و الرنين المغناطيسي بالنسبة إلى درجة نشاط المرض و نتائج الفحوص المعملية و قد وجدت علاقة إحصائية موجبة بين درجة نشاط المرض و زيادة درجة إصابة المفصل.
كما وجدت أيضا علاقة إحصائية موجبة بين إصابة الحنجرة بالمرض و درجته الوظيفية و التي تدل على درجة تقدمه.
و من ذلك يمكننا أن نستخلص أنه من الضروري فحص الحنجرة في مرضى الرثيان المفصلي روتينيا باستخدام المنظار الليفي الأنفي الحنجري للاكتشاف المبكر للمرض أو في مرحلة يمكن علاجها.
و لما كان للرنين المغناطيسي حساسية عالية في تحديد أي إصابة مبكرة و دقة المسح الإشعاعي لتقييم الأنسجة الرخوة فننصح به في حالات معينة و التي تحتاج إلى وسيلة عالية الكفاءة لإظهار الإصابة المبكرة و من ثم سرعة العلاج قبل حدوث أية مضاعفات.
Abstract EN
Objective: The cricoarytenoid (CA) joint is a true diarthrodial joint that can be affected with rheumatoid disease.
Its strategic location in the airway anatomy makes its evaluation of clinical importance.
Materials & Methods: Forty-eight rheumatoid arthritis patients (RA) who have been attending the Rheumatology and Rehabilitation Department of Assiut University Hospitals were subjected to direct nasofibrolarynogoscopy (NFL) at the ENT Department and Phoniatrics Uunit as well as magnetic resonance imaging (MRI) to assess the larynx.
Results: Laryngeal involvement was detected with NFL and MRI in 33% of patients.
There was no significant correlation between the disease duration and the individual parameters of clinical scores (morning stiffness, joint score, pain scale, grip strength, ESR, CRP) and NFL or MRI findings.
There was a significant statistical relation between visualized lesions with NFL and MRI, and both disease activity and functional class.
Recommendations: NFL examination is to be performed routinely for RA patients while MRI can be helpful to elucidate an early accurate diagnosis in certain RA patients who are suspected to have CA involvement, allowing early management.
