الهدف : أن الهدف من هذه الدراسة هو تقييم مدى حدوث هبوط التوتر الانتصابي في مرضى السكتة الدماغية الذين يجتازون التأهيل و تحديد المتغيرات الإكلينيكية المرتبطة به.
الطريقة : و قد اشتملت الدراسة ثلاثين مريضاً بالسكتة الدماغية لأول مرة و الذين يجتازون التأهيل في خلال شهر واحد من بداية السكتة الدماغية، و كانوا 17 من الذكور و 13 من الإناث، و تراوحت أعمارهم بين 49 و 68 عاما، و قد تم أخذ تاريخ المرض كاملا، و عمل فحص إكلينيكي شامل و عمل فحوصات للمرضى، و قد تم قياس ضغط الدم للمريض و هو مستلق على ظهره و هو مائل عند تسعين درجة.
و قد تم تعريف هبوط التوتر الانتصابي أنه هبوط في ضغط الدم الانقباضي بمقدار عشرين ميلليمتر زئبق أو أكثر.
النتائج : و قد أظهرت النتائج وجود هبوط التوتر الانتصابي في 14 (46.7 %) مريض.
كما أظهرت النتائج وجود علاقة ذات دلالة إحصائية عالية بين كل من هبوط التوتر الانتصابي و العمر و وجود علاقة ذات دلالة إحصائية بين كل من هبوط التوتر الانتصابي و الحالة الوظيفية المنخفضة للمريض عند بداية اجتيازه للتأهيل.
و قد اظهرت النتائج وجود علاقة غير ذات دلالة إحصائية بين كل من هبوط التوتر الانتصابي و مكان السكتة الدماغية أو مهبطات الضغط العالي.
الخلاصة : و خلاصة هذه الدراسة أن هبوط التوتر الانتصابي كان شائعا في مرضى السكتة الدماغية الذين يجتازون التأهيل و خصوصا في المرضى الأكبر في السن الذين تكون حالتهم الوظيفية منخفضة.
Abstract EN
Objective : to assess the incidence of orthostatic hypotension (OH) in stroke patients undergoing rehabilitation and to determine clinical variables associated with it.
Methodology : Thirty patients with a first clinical stroke who underwent rehabilitation within one month of stroke onset were included in this study.
They were 17 males and 13 females.
Their age ranged from 49 to 68 years (mean + SD = 58.1 + 6.3).
Patients were subjected to full history taking, thorough clinical examination and investigations.
Patients’ blood pressure was measured in the supine position and on tilting at 90.
OH was defined as a drop in systolic blood pressure (SBP) of 20 mmHg or more.
Results : OH was present in 14 (46.7 %) patients.
Age was highly significantly associated (p < 0.001) with OH and admission modified Barthel Index (MBI) score was significantly associated (p < 0.05) with OH.
There was a non-significant correlation with the site of stroke or the use of anti-hypertensive.
Conclusion : OH was common in stroke patients undergoing inpatient rehabilitation especially in older patients who have a lower functional status.
