الهدف : دراسة الأجسام المضادة للكيراتين في مصل مرضى الرثيان المفصلي و توضيح وجود علاقة ارتباطية بينه و بين عامل الرثيان و المقاييس الإكلينيكية و المعملية لنشاط المرضى و كذلك علاقته بوجود التآكل العظمي و الأعراض غير المفصلية للمرض، و كذلك دراسة تأثير كل من الأدوية المضادة للالتهاب غير الاسترودية (ديكلوفيناك صوديوم) و الكورتيزونات (بريدنيزولون) و كذلك الأدوية المحورة للمرض (ميثوتريكسات) على نسبة للأجسام المضادة للكيراتين بمصل الدم.
الطريقة : أجريت هذه الدراسة على 45 مريضا بمرض الرثيان المفصلي و قد تم تقسيمهم إلى 3 مجموعات كل مجموعة تشمل 15 مريضا حسب طريقة العلاج : المجموعة الأولي : تناولت ديكلوفيناك صوديوم 100 مجم يوميا لمدة 3 شهور.
المجموعة الثانية : تناولت بريدنيزولون 10-20 مجم يوميا لمدة 3 شهور.
المجموعة الثالثة : تناولت ميتوتريكسات 7.5–15 مجم / أسبوع و حمض الفوليك 1 مجم يوميا لمدة 3 شهور.
و قد خضع كل المرضي إلى الآتي عند بداية و نهاية البحث : تاريخ مرضي كامل، فحص اكلينيكيو فحص معملي و يشمل : صورة دم كاملة، سرعة ترسيب، بروتين سي التفاعلي، عامل روماتويد و الأجسام المضادة للكيراتين.
و تم كذلك عمل أشعة عادية لليدين لكل المرضى.
النتائج : و قد أظهرت نتائج البحث ما يلي : 1- توجد الأجسام المضادة للكيراتين في مصل مرضى الرثيان المفصلي بنسبة 68.9 %.
2- توجد علاقة ارتباطية بين الأجسام المضادة للكيراتين و بين المقاييس الاكلينيكية و المعملية لنشاط المرض كما يوجد ارتباط بينها و بين عامل الرثيان و بروتين سي التفاعلي بالدم في مرضى الرثيان المفصلي.
3- انخفاض مستوى الأجسام المضادة للكيراتين بالدم بعد العلاج بكل من الميثوتريكسات و البريدنيزولون و هذا الانخفاض متناسب مع التحسن في المقاييس الاكلينيكية و المعملية لنشاط المرض.
4- عدم وجود ارتباط بين الأجسام المضادة للكيراتين و وجود التآكل العظمي بمرض الرثيان المفصلي.
و يوصي هذا البحث بقياس الأجسام المضادة للكيراتين و اعتبارها أحد الاختبارات المعملية الأساسية لتشخيص مرض الرثيان المفصلي كما يمكن استخدامها لمتابعة نشاط المرض و استجابة المريض للعلاج.
Abstract EN
Objective : To assess the ant keratin antibody (AKA) of IgG class in the sera of rheumatoid arthritis (RA) patients and its correlation to rheumatoid factor, disease activity, bone erosion and extra-articular manifestations.
Also, the effect of different ant rheumatic drugs namely diclofenac sodium as a non-steroidal anti-inflammatory drug (NSAID), prednisolone and methotrexate as a disease modifying anti-rheumatic drug (DMARD) on IgG AKA titer in RA patients was assessed.
Methodology : this study was carried out on 45 RA patients diagnosed according to the American College of Rheumatology (ACR) 1988 criteria.
The patients were divided into 3 groups (15 patients each) according to the line of treatment: Group I: received diclofenac sodium 100 mg/day for 3 months.
Group II : received prednisolone 10-20 mg / day for 3 months.
Group III : received methotrexate 7.5-15 mg / week plus folic acid 1mg / day for 3 months.
Erythrocyte sedimentation rate, CRP, RF and ant keratin antibody were determined in all patients at the start and at the end of the study.
Also, plain x-rays of both hands were done for all patients at study entry.
Results : Ant keratin antibody was found in 68.9 % of RA patients and it correlated with parameters of disease activity, RF, CRP and subcutaneous nodules.
There was a significant reduction in AKA titer after treatment with MTX and predni-solone.
The best reduction was in MTX group and it correlated with improvement in parameters of disease activity.
There was no correlation between AKA and degree of bone erosion.
Conclusion : the sensitivity of AKA in RA patients was 68.9 %.
AKA should be included in the initial investigations for the diagnosis of RA and it can be used as a prognostic marker for the disease and in evaluating the effect of treatment.
