هدف البحث : إن نتائج الدراسة التشخيصية الكهربائية لها نتائج إيجابية كاذبة و سلبية كاذبة علاوةً عن كونها إجراء غالٍ و مكلف.
يهدف هذا البحث إلى تقييم ضرورة إجراء هذه الدراسة التشخيصية قبل جراحة تحرير النفق الرسغي عند المرضى ذو التشخيص الواضح سريرياً و مدى قدرة هذه الدراسة على التنبؤ بنتائج الجراحة.
طرق البحث : تم إجراء دراسة مستقبلية مقارنة شملت 207 مرضى مشخصين بمتلازمة النفق الرسغي (متوسط أعمارهم 42.35 ± 11.19 سنة).
تم توزيع الحالات إلى مجموعتين : تم تأكيد التشخيص في المجموعة الأولى بإجراء دراسة تشخيصية كهربائية (100 حالة)، بينما اعتمد التشخيص على الموجودات السريرية فقط في المجموعة الثانية (107 حالات).
كانت المجموعتان السابقتان متقاربتين من حيث العمر، الأعراض الملاحظة، مدة الشكاية و الموجودات الفيزيائية.
خضع المرضى في كلتا المجموعتين لإجراء جراحة مفتوحة لتحرير النفق الرسغي و تم تقييم نتائج الجراحة بعد أسبوعين و ستة أشهر من قبل طبيب غير مطلع إلى أي من المجموعتين يعود المريض.
النتائج : كانت الاستجابة للجراحة متقاربة بين المجموعتين (ممتازة عند 91 %، مقبولة عند 5.8 %، و سيئة عند 4.8 % من مرضى المجموعة الأولى مقارنة مع 87.9 %، 5.6 %، 6.5 % على الترتيب من مرضى المجموعة الثانية، p = 0.493).
الاستنتاجات : يمكن الاستغناء عن إجراء الدراسة التشخيصية الكهربائية في الحالات الواضحة سريرياً من متلازمة النفق الرسغي و ذلك حتى عند إحالة المريض للجراحة.
Abstract EN
Objective : Electrodiagnostic study has false positive and false negative results, moreover it is invasive and costly.
This research aimed to evaluate if this study is obligatory before carpal tunnel release surgery in patients with clinically evident diagnosis, and if this study can predict surgical outcome.
Methods : A prospective comparative study included 207 cases (mean age±standard deviation 42.35±11.19 years) diagnosed with carpal tunnel syndrome.
One hundred cases had undergone confirmatory electrodiagnostic study (Group 1) while 107 had not (Group 2).
Patients in both groups were comparable for age, symptoms, duration of complaint, and physical findings.
Both groups underwent open carpal tunnel release surgery and were evaluated two weeks and six months later by a physician not aware to which group the patient belonged.
Results: Response to surgery was comparable between the two groups (excellent in 91%, acceptable in 5.8%, and bad in 4.8% in the first vs.
87.9%, 5.6%, and 6.5% respectively in the second, p=0.493).
Conclusions : Electrodiagnostic study could be omitted in clinically evident carpal tunnel syndrome, even when patient is referred to surgery.
