مقدمة : التهاب العظم و النقي القيحي المزمن في العظام الطويلة هو التهاب شائع وصعب العلاج.
الأهداف : كان الهدف من هذه الدراسة هو دراسة نمط الأعراض و نتائج علاج التهاب العظم و النقي القيحي المزمن في العظام الطويلة في المستشفيات المتخصصة في نيجريا.
المرضى و الطرق : تمت مراجعة و تحليل سجلات المرضى الذين عولجوا من التهاب العظم و النقي القيحي المزمن بين يناير 2009 و ديسمبر 2011 في وحدات (NKST) بمستشفى مكار التأهيلي بأثر رجعي للعمر، الجنس، العظام المصابة، العزلات الميكروبيولوجية، طرق العلاج و تكرار الإصابة.
النتائج : تمت دراسة ثلاثة و خمسين من المرضى الذين يعانون من التهاب العظم و النقي القيحي المزمن في 57 من العظام الطويلة.
30 من الذكور (56.6 %) و 27 من الإناث (43.4 %)، نسبة الذكور إلى الإناث من 1.1 : 1.
تراوحت الأعمار بين 3-60 سنة (متوسط عمري 20.34 ± 13.48).
و كان سوء معالجة أو إهمال التهاب العظم و النقي الحاد المنتشر عن طريق الدم سبب رئيسي لالتهاب العظم و النقي المزمن في 40 مريض (70.2 %).
و شملت العظام المصابة الساق في 29 (50.9 %)، عظم الفخذ 11 (19.3 %) و العضد 9 (15.8 %) من الحالات.
كانت المكورات العنقودية الذهبية الميكروبات المعزولة الأكثر شيوعا 13 (52 %).
أهم العمليات الجراحية كانت إزالة الشظايا و الكحت حيث أجريت ل 51 (96.2 %) من الحالات.
الخلاصة : التهاب العظم و النقي المزمن هو في الغالب مرض الأطفال و يؤثر على الساق في الغالب.
سوء معالجة أو إهمال التهاب العظم و النقي الحاد المنتشر بالدم هو السبب الرئيسي للمرض.
Abstract EN
Chronic pyogenic osteomyelitis of long bones is common and difficult to treat.
Objectives : The aim of this study was to examine the pattern of presentation and outcome of treatment of chronic osteomyelitis of long bones at specialized hospital in Nigeria.
Patients and methods : Case records of patients who were managed for chronic osteomyelitis between January 2009 and December 2011 at Nongu u Kristu u I Ser u sha Tar (NKST) Rehabilitation Hospital, Mkar, were retrieved from the Medical Records Department and analyzed retrospectively for age, gender, bones involved, microbiological isolates, treatment modalities and recurrence.
Results: Fifty-three patients with chronic pyogenic osteomyelitis of 57 long bones were studied.
