خلفية : التهاب اللوزتين البلعومي يمثل مشكلة رئيسية للصحة العامة في جميع أنحاء العالم.
و قد كشفت الدراسات الحديثة في السودان نتائج مثيرة للقلق عن تعاطي كبير للمضادات الحيوية دون وصفة طبية لعلاج التهاب اللوزتين الحاد.
الأهداف : كان الهدف من هذه الدراسة هو معرفة انتشار المجموعة بيتا الحالة للدم من السبحيات العقدية و توثيق مدى ملاءمة استخدام المضادات الحيوية في علاج التهاب اللوزتين البلعومي الحاد في مستشفى جعفر بن عوف للأطفال.
المواد و الطرق : أجريت دراسة مقطعية أساسها المستشفى في الفترة من يناير حتى أغسطس عام 2012 في مستشفى جعفر بن عوف٬ أكبر مستشفى تخصصي لعلاج الأطفال في السودان.
و كان حجم العينة 100 بما في ذلك الأطفال الذين تتراوح أعمارهم بين 2-17 سنوات و حصلوا على علاج المضادات الحيوية لالتهاب الحلق الراهن.
و قد تم جمع مسحة البلعوم عند حضور الأطفال بهذا الالتهاب قبل بدء العلاج.
و قد تم صبغ المسحات بصبغة الغرام أولا٬ و من ثم تم عزل المجموعة الأولى من العقديات الحالة للدم بيتا و التي تم تحديدها في المختبر من خلال خصائص نموها.
و جرى تقييم جميع الأطفال المشمولين في الدراسة سريريا و من خلال استبيان معد لذلك الغرض.
و قد تم تحليل البيانات عن طريق البرنامج الإحصائي SPSS الإصدار 19.
النتائج : كان الانتشار المتحصل عليه من التهاب اللوزتين بمجموعة بيتا الحالة للدم العقدية و / أو التهاب البلعوم 22 ٪٬ و مع ذلك كانت نسبة الوصفة الطبية للمضادات الحيوية بنسبة 100 %.
الاستنتاج : تم علاج معظم الأطفال بشكل غير لائق فيما يتعلق بالحاجة إلى استخدام المضادات الحيوية٬ و نوع المضادات الحيوية المستخدمة و مدة العلاج.
كما اقترحت العديد من الدراسات أن زيادة استخدام المضادات الحيوية قد يكون راجعا إلى عدم التيقن من التشخيص٬ فإن وضع نظام نقطي للتشخيص أو وجود وسيلة مختبرية للتشخيص السريع يمكن أن تسهم في الحد من معدل وصف المضادات الحيوية.
Abstract EN
Pharyngo-tonsillitis represents a major public health problem all over the world.
Recent studies in Sudan have revealed alarming results reporting antibiotics over prescription in the management of acute tonsillitis.
Objectives : The aim of this study was to estimate the prevalence of Group A Beta hemolytic streptococci and document the appropriateness of using antibiotics in the treatment of acute tonsillo-pharyngitis in Jaafar Ibn Auf Pediatrics Hospital.
Materials and Methods : A cross sectional hospital based study was conducted in the period January to August 2012 in Jaafar Ibn Auf Hospital, the largest tertiary pediatrics hospital in Sudan.
The sample size was 100 including children aged 2-17 years and got antibiotics treatment for their current sore throat.
A pharyngeal swab was collected at presentation from tonsils and posterior pharynx.
Gram staining was done first, and then Group A Beta hemolytic streptococci were isolated and identified in the laboratory by their growth characteristics.
All children included in the study were assessed clinically and subjected to a structural questionnaire.
Data were analyzed by SPSS version 19.
Results : The estimated prevalence of Group A Beta hemolytic streptococci tonsillitis and/or pharyngitis was 22 %, nevertheless the proportion of antibiotic prescription was 100 %.
Conclusion : Most children were treated inappropriately regarding the need for using antibiotics, the type of antibiotics used and the duration of management.
As many studies suggested that increased using of antibiotics may be due to uncertainty of diagnosis, requesting scoring system or rapid diagnostic test can contribute to the reduction of the rate of antibiotics prescription.
