Abstract AR
أجريت هذه الدراسة بمستشفي أسيوط الجامعي بقسم الجراحة العامة لتقييم تطويل الفك السفلي عن طريق الشد.
و قد أجريت هذه الدراسة علي أربعة مرضي، ثلاثة إناث و ذكر، تتراوح أعمارهم من ٩ إلى 22 سنة و الذين يعانون من تشوه بالوجه في صورة رجوع الفك السفلي للخلف في ثلاثة حالات و رجوع و اعوجاج الفك السفلي في الحالة الرابعة.
تم اخذ تاريخ مرضي كامل لجميع المرضي و تم فحصهم إكلينيكيا و أجريت لهم التحاليل اللازمة و أيضا تم عمل الأشعات المطلوبة و هي عبارة عن أشعات على الفك السفلي منظر تاون و منظر خلفي أمامي و جانبي و أشعة بانوراما على الفكين و أشعة سيغالومتري جانبي قبل و بعد أجراء الجراحة اللازمة و في المتابعة للمريض كانت نتائج هذه الدراسة على النحو التالي :- وجدت زيادة ملحوظة في طول الفك بعد انتهاء الشد مع تحسن مظهر الوجه لجميع الحالات و قد لوحظ عدم وجود إصابة للعصب السفلي للأسنان لكن لوحظ حدوث عميل بالشفة السفلي أثناء الشد و قد تحسنت بعد أنتها فترة الشد.
كذلك ثم تسجيل أن شكوى عائلات المرضي من تشخير المرضي أثناء النوم قد اختفت تماما بعد انتهاء الشد.
أما عن الأعراض الجانبية فكانت رحالتين انتكاسية ضئيلة في طول الفك السفلي بمد فترة هن انتهاء الشد و كنلك حدث في حالة واحدة التهاب حول المسمار بالجلد و لكن تم علاجه بواسطة مضاد حيوي موضعي).
نستخلص من هذه الدراسة أن تطويل الفك ألسفلي عن طريقة سهلة مع وجود نسبة ضئيلة من الأعراض الجانبية.
و يمكن للمريض البقاء خارج المستشفي مع متابعة أسبوعية مما يقلل من تكاليف العملية.
و كذلك يجب أن يقوم جراح الوجه و الممكن و أخصائي تقويم الآسنان بالعمل معا كفريق واحد حتى تكتمل النتائج المرجوة من تطويل الفك السفلي.

The.
results of this study showed significant increase of the mandibular length after distraction with improvement of/he facial appearance of the patients.
In all cases their wits no injury of the inferior alveolar nerve, but lower lip numbness- and hypothesia occurred with distraction and disappeared after stoppage of distraction Also the families of our case' noted marked improvement of snoring and it was Completely disappeared after stoppage of' distraction.
The side effects of distraction osntogenesis of the mandible were very minimal in the form of the occurrence impartial relapse in two cases and infection around the pin track occurred m one case only and controlled by local antibiotic.
In conclusion, mandibular lengthening by distraction osteogenesis, using an extra-oral distraction device, is very effective and is a simple technique with minimal side effects, apart from the residual skin scars The openstion can be performed on an outpatient basis, requiring only weekly controls, considerably decreasing hospital expenses.
The maxillofacial surgeons and the orthodontists must work in a team t>> complete this work and gain the best results.
