الهدف من هذه الدراسة هو تقييم كفاءة العلاج الكيميائي باستخدام عقاري السيسبلاتين و الأيتوبوسيد عند أعطالهم كعلاج أولى (قبل الإشعاع) للأطفال الذين يعانون من ورم متبقي بعد الجراحة لأورام المخيخ.
تم دراسة 15 طفلا مريضا ممن شخصوا تشخيصا مؤكدا بورم بالمخيخ بعد إجراء عينة باثولوجية و تم علاجهم بثلاث دورات من السيسبلاتين بجرعة 100 ملجم / م حقن وريدي يوم و الأيتويوسيد بجرعة 100 ملجم / م حقن وريدي لمدة 3 أيام ثم تم إعطاء العلاج الإشعاعي للمخ و النخاع الشوكي و بعد ذلك تم إعطاء 3 دورات من العلاج الكيميائي مرة أخرى.
بعد 3 دورات من العلاج الكيميائي المبدئي كان هناك 3 مرضى حدث لهم شفاء كامل، 9 أطفال حدث لهم استجابة جزئية و 2 مرضى لم يستجيبو للعلاج و طفل واحد حدث له ازدياد في حجم الورم.
بعد استكمال العلاج الإشعاعي للمخ و النخاع الشوكي، كان هنالك 11 طفلا حدث لهم استجابة كليه للعلاج و 4 أطفال حدث لهم استجابة جزئية للعلاج عند استكمال 3 دورات علاج كيميائي أخرى كانت النتائج النهائية هي 12 طفلا باستجابة كلية للعلاج و 3 أطفال لهم استجابة جزئية.
تراوحت الفترة التي لم يحدث فيها انتكاس للمرضى بين 4-22شهر (بمدى أوسط 1، 14 شهر).
الأعراض الجانبية للعلاج كانت قليلة جدا في صوره غثيان بسيط لمده 4،2يوم بعد العلاج الكيميائي انخفاض الدم الأبيض حدث ل5 مرضى (5 دورات من 90 دورة تم إعطاءها) و لم يكن هناك أي أعراض للتأثير على وظائف الكلي أو العصب السمعي خلال هذه الدراسة.
نستنتج من هذه الدراسة أن نظام العلاجي المستعمل قد أعطي نتائج عالية و مشجعة و بنسبة ممتازة و لأن عدد المرضى كان قليلا نسبيا فإننا ننصح بمتابعة البحث بهذا النظام العلاجي و متابعة المرضى فترة أطول.
Abstract EN
Aim: To evaluate the efficacy of per-irradiation chemotherapy (cisplatin VP 16) in children with residual postoperative medulloblastorna Patients and medthods : fifteen children with a pathologically confirmed diagnosis of medullobastoma were treated with 3 cycles of cisplatin 1OOmg/m2IV.
DL and VP 16 100mg/m2 IV.
DL-3 befor standard craniospinal Irradiation than Followed by another 3cycles chemotherapy after irradiation.
Result : After the initial I cycles of Cisplatin-VP 16, there were 3«complete : responders (CR), 9 partial responders (PR).
2 patients with stationary disease( SD) and one patient had a progressive disease (PD).
After completing the craniospinal irradiation .
there were II complete responses and 4partial responies Al the end of '6 cycles , there were 12 CRs and 3 PRs .
The progression-free survival ranged between 4 and 22moths (mea n 1 4.1 months).
The toxicity to treatment was minimal ranging from mild to moderate.
Nausea for 2-4 days post chemotherapy.
Grade 1 neutropenia was encountered in .5 patients (5 in 90 cycles).
No renal or ototoxicity ii ere encountered during the study.
Conclusions: Although the number of studied patients were small.
The high response rate and low toxicity associated with per-irradiation chemotherapy is encouraging and long-term survival will be evaluated.
