Spontaneous biliary duct perforation is a rare yet important entity presenting in infants.

The usual presentation is jaundice and failure to thrive occurring in a previously healthy infant.

The diagnosis is usually the function of hepatobiliary imaging.

In this case report Tc99m-DISIDA hepatobiliary imaging was an accurate tool to diagnose this surgically correctable disease.
