يعتبر تناذر الشريان المساريقي العلوي حالة نادرة جدا، و هو اضطراب وعائي هضمي مهدد للحياة يتميز بانضغاط الجزء الثالث من العفج بالشريان الأبهر البطني و الشريان المساريقي العلوي من الأعلى.
سيتم هنا عرض حالة سيدة عمرها 25 سنة راجعت بنوبات متكررة من الألم الشرسوفي، توسع في البطن مع إقياء تفاقمت بعد خسارة وزن كبيرة ناجمة عن اتباع حمية غذائية صارمة.
أظهر التصوير الهضمي العلوي الظليل بالباريوم توسعا في المعدة والجزء الثاني من العفج مع توقف عبور المادة الظليلة في الجزء الثالث منه، دون ملاحظة أية تغيرات في المخاطية.
تبين بإجراء التصوير الطبقي المحوري وجود تضيق في الزاوية ما بين الأبهر البطني و الشريان المساريقي العلوي مع نقص المسافة بينهما، و بالتالي انضغاط في الجزء الثالث من العفج.
تمت تجربة المعالجة المحافظة عند المريضة لمدة ستة أشهر إلا أنها فشلت.
يمكن معالجة هذا التناذر بشكل محافظ، إلا أن العلاج الجراحي بإجراء مفاغرة عفجية صائمية عبر مساريقا الكولون المعترض هو الأكثر فعالية.
Abstract EN
Superior mesenteric artery (SMA) syndrome is a very rare, life-threatening gastro-vascular disorder characterized by a compression of the third portion of the duodenum by the abdominal aorta (AA) and the overlying superior mesenteric artery.
We describe a case of a 25 year old lady who presented with recurrent attacks of epigastric pain, abdominal distension, and vomiting that was aggravated by weight loss following some weight reduction dietary program.
Upper gastrointestinal study with gastrograffine showed a dilated stomach and second part of the duodenum with cut off at the third part of duodenum with no intrinsic mucosal abnormalities.
CT scan of the abdomen revealed a narrow angle and decreased distance between the aorta and superior mesenteric artery, thus compressing the third duodenal part.
Conservative treatment was tried for six months but failed.
This condition can be treated medically but surgical correction with a transmesocolic duodenojejunostomy is more effective.
