التناقض بين أشعة الفحص النووي للعظام و مسح التصوير المقطعي لمنطقة عظمية متفردة في الأشعة النووية في المرضى الذين يعانون من سرطان الثدي يشكل تحديا للأطباء.
الغرض من هذه الدراسة هو تقييم خطر انتشار السرطان لمنطقة منفردة في العظام اكتشفت في الأشعة النووية في المرضى الذين لديهم سرطان الثدي في مستشفى جامعه الملك عبد العزيز، و كان هناك ٨٩ مريضا قد لاحظت تغيرات في عظمة منفردة في الأشعة النووية و قورنت بالتصوير المقطعي الذي تم في غضون ٣ أشهر و كانت التغيرات العظمية الانفرادية قي ٥٦ مريضا ٦٩ مريضا (63 %) و خبيثة في ٣٣ مريضا (37 %).
كان هناك 15 انتشار عظمي خبيث (17 %) في فحص العظام النووي التي كانت نتائج التصوير المقطعي الأولية التي كانت سلبية أو مبهمة و لكنها كانت واضحة كانتشار سرطاني عظمي في متابعة المسح المقطعي و باختصار، فإن نسبه الانتشار الخبيث عالية (٣٧ %) في التغيرات العظمية المنفردة في المسح النووي في المرضى الذين يعانون من سرطان الثدي في مركزنا الطبي الجامعي، بغض النظر عن ظهورها واضحة على مسح التصوير المقطعي الأولي.
لذا، يصح بمزيد من التقييم و دراسة مستقبلية لعدد أكبر من المرضى.
Abstract EN
The discrepancy between bone scintigraphy and computed tomography scanning for solitary bony lesion in patients who have breast cancer is challenging to the referral physician.
The purpose of this study was to evaluate the risk of malignancy in solitary lesions on bone scintigraphy in patients who had breast cancer at King Abdulaziz University Medical Centre, and to compare the results between bone scintigraphy and computed tomography scanning.
There were 89 patients who had a solitary bone lesion noted on bone scintigraphy and computed tomography performed within 3 months of bone scintigraphy.
^e solitary bone lesions were benign in 56 (63%) patients and malignant in 33 patients (37%o).
There were 15 (17%0ر malignant lesions in bone scan that had initial computed tomography findings that were negative or e^ivocal for bone metastasis, but all these lesions had destructive changes on follow-up computed tomography scan.
In summary, at this medical center the fre^ency of malignancy is high (37 %) in solitary bone lesions in patients who have breast cancer, regardless of appearance of the lesion on an initial computed tomography scan.
Prospective study with a larger group of patients is recommended.
