تعتبر عملية إصلاح الفتق الإربي أكثر العمليات شيوعا و بالتالي فإن فشل هذه العملية يؤدي إلى عدم ارتياح المريض و إعادة إجراء العملية و إلى العبء الاقتصادي على كاهل الفرد و المجتمع.
و قد أجريت هذه الدراسة في الفترة من إبريل 2004 م إلى نوفمبر 2008 م على 200 مريضا يعانون من فتق اربي غير مرتجع و تم تقسيم هؤلاء المرضى إلى مجموعتين في المجموعة أ تم إصلاح الفتق بدون استخدام شبكة و تم استخدام شبكة في إصلاح الفتق في المجموعة ب و تم متابعة هؤلاء المرضى أثناء وجودهم بالمستشفى و بعد كل 6 أشهر لمدة سنتين و فعليا لم يخضع للمتابعة سوى 81 مريضاً و قد وجد انه في مرضى المجموعة ب لم يحدث أي ارتجاع للفتق و على الرغم من أن زمن إجراء العملية كان أطول في المجموعة ب إلا أن وقت مكوثهم بالمستشفى كان أقل كما أن الألم بعد العملية و المضاعفات كانت أقل و قد رجع مرضى المجموعة ب إلى عملهم في فترة أقل من مرضى المجموعة أو نستخلص من هذه النتائج أن علاج الفتق الإربي الأولي في المرضى البالغين باستخدام الشبكة هو الأفضل و يجب اتخاذ هذا المبدأ في كل المرضى البالغين الذين يعانون من الفتق الإربي الأولي.
Abstract EN
Background : Inguinal hernia is the most frequently performed operation.
Consequently, failure of inguinal hernia repair has a great impact on society and leads to increased patient discomfort, reoperations and considerable economic burden.
The aim of this study was to compare the result of repair of primary inguinal hernia using mesh with the usually performed Bassini repair.
Methods : Between April 2004 and November 2008, 200 patients with unilateral primary inguinal hernia were entered into the: study patients were distributed into 2 groups A and B.
In group A patients, Bassin repair was performed.
In group B mesh repair was performed.
The patients were followed up during their hospital stay and every 6 month for 2 years.
The main follow up variables were operating time, duration of hospital stay, postoperative pain, complications, return to normal activities and recurrence rate.
Results: Only 81 patients have received follow up examinations.
In group B no recurrence occurred (0% versus 4.7 %).
Although the mean operating time was longer in group B (56.5 versus 43.7 minutes), the mean duration of hospital stay was shorter (21.8 versus 25.6 hours) postoperative pain and complications were less and return to normal activities was earlier (12.3 versus 23.1 day) than in group A.
Conclusion: Mesh inguinal hernia repair is superior to the usually preformed Bassini repair and should be adopted as standard treatment of all adult primary inguinal hernia.
