لكي يتم التميز بين نوبات الأحداث الصرعية و الغير صرعيه عادة يتم استخدام رسام المخ الكهربائية المصاحب بالفيديو لمدة طويلة حتى يتسنى تسجيل نوبة أو أكثر مما يسمح بتحليل أحداث هذه النوبة إكلينيكيا و كهربائيا من خلال تحليل الفيديو و رسم المخ معا.
مثل هذا الفحص يتطلب الكثير من الجهد و المال و الوقت لكي يتم عمله بشكل صحيح لمرضى القسم الداخلي تحديدا.
لذا كان من المفترض أن تتم دراسة فاعلية رسم المخ المصاحب بالفيديو لمدة زمنيه قصيرة لا تتعدى الساعتين و هو ما تم في هذا البحث.
بناءا على معلومات و دراسات سابقة حول هذا الموضوع يتضح أن رسم المخ المختصر كان جيدا في تشخيص العديد من الأمراض دون اللجوء إلى استخدام رسم المخ طويل المدة.
لكن معظم الدراسات السابقة كانت تستخدم رسم المخ لمدة لا تقل عن أربع ساعات.
في الدراسة، قمنا بعمل رسم المخ لمدة تصل إلى ساعتين بحد أقصى حتى نتمكن من تحقيق نفع مادي أكبر و توفير أكثر للجهد المبذول.
هذه الدراسة اشتملت على ستة و ثلاثين مريضا يعانون من نوبات مختلفة و قد خضعوا لرسم المخ الكهربائي المصاحب بالفيديو لمدة ساعتين مما أدى إلى الوصول إلى حقيقة التشخيص الواحد و عشرين مريض منهم الخمسة عشر مريضا الآخرين لزم استكمال رسم المخ لهم لمدة زمنية أطول حتى يتم الوصول إلى تشخيص صحيح لكل منهم.
باستخدام رسم المخ الكهربائي المصاحب بالفيديو لمدة ساعتين فقط أستطاع فريق البحث تشخيص أحد عشر مريضا مصابا بنوبات نفسيه لا صرعيه، و خمسة مرضى مصابين بنوبات صرعيه حقيقية و خمسة آخرون مصابين بنوبات اختلال الحركة و الاضطراب بضربات القلب و هذا يعني أن استخدام رسم المخ لمدة قصيرة تصل إلى الساعتين يفيد في تشخيص مثل هذه الحالات دون الحاجة إلى استخدام هذا الفحص لمدة طويلة و مثل هذا الأمر يوفر الكثير من الجهد و المال و الوقت.
Abstract EN
Background : to distinguish epileptic seizures from non-epileptic paroxysmal events, we often use video electroencephalography (EEG) recording.
Long term video electroencephalography recording for inpatients is of good value, but, it necessitates much resources (financial and manpower).
There is some evidence suggesting that short term video EEG (Stveeg) is useful, but its role in practice has yet to be evaluated.
Objective : to assess the usefulness of STVEEG in the diagnosis of different paroxysmal disorders and classifying epilepsy if present.
Methods : Thirty sex patients had STVEEG recordings (for 2 hours) during a 15 months period.
Results : A diagnostic event was recorded in 21 of 36 (58.3 %) within the first 2 hours of monitoring : PNES (n =11), epilepsy (n = 5), and other paroxysmal disorders e.g.
syncope, hemi facial spasm, movement disorder (n = 5).
Conclusions : Stveeg is a useful diagnostic technique in diagnosis of different paroxysmal disorders and it can be used as a successful screening test for saving other resources like prolonged inpatient video EEG recording.
