موضوع البحث : یعتبر مفھوم ”ما وراء المعرفة" عاملا مھما في تطور و علاج مختلف الاضطرابات النفسیة، خاصة في اضطراب الوسواس القھري (OCD) .و قد كان الھدف من الدراسة الحالیة استكشاف الدور الذي یلعبھ ھذا المفھوم ”ما وراء المعرفة" في التسبب في أعراض الوسواس القھري في عینة من المصریین.
طرائق البحث : تم اختیار أربعین مریضا ممن یعانون من الوسواس كمجموعة ضابطة متجانسة إحصائیا صحیحا القھري و ستة و أربعون شخصا مع المرضى في متغیرات السن و الجنس و المستوى الاقتصادي الاجتماعي.
و تم تقییم أعراض الوسواس القھري باستخدام مقیاس ییل براون للوسواس، كما تم تعریب و حساب الخصائص السیكومتریة و تطبیق استبیاني اتجاھات المسؤولیة و ما وراء المعرفة – نسخة العبارات الثلاثین.
النتائج : سجلت مجموعة المرضى المصابین بالوسواس القھري درجات أعلى بكثیر من المجموعة الضابطة في جمیع الأبعاد الفرعیة، بالإضافة إلى النتیجة الكلیة، لاستبیان ما وراء المعرفة – 30 .و تمیزت الأبعاد الخاصة بالثقة المعرفیة و المخاطر و السیطرة، بالإضافة إلى النتیجة الإجمالیة، بأحجام تأثیریة مرتفعة.
و قد تم تسجیل تناسب طردي قوي بین أبعاد ما وراء المعرفة و بین شدة أعراض الوسواس القھري.
و عند استبعاد التأثیر الخاص بالشعور بالمسئولیة عن الأفكار فإن معظم الأبعاد الخاصة بما وراء المعرفة قد احتفظ بعلاقتھ الطردیة الدالة بشدة الوسواس و لم یفقد ھذه العلاقة سوى في بعدین من أبعاد ما وراء المعرفة.
خلاصة البحث : یرتبط الاختلال في مفاھیم ما وراء المعرفة بأعراض الوسواس القھري.
و قد یؤدي فھمنا لھذه العلاقة إلى صیاغة جدیدة للبناء المعرفي لمرض الوسواس القھري مما یفید في تطویر علاجات معرفیة جدیدة لھذا المرض.
Abstract EN
Metacognition has been considered an important factor in the development and maintenance of various psychiatric disorders, especially in Obsessive-Compulsive Disorder (OCD).
The aim of the current study was to explore the role of metacognition in the pathogenesis of obsessive-compulsive symptoms in an Egyptian sample.
Methods: Forty patients with OCD and 46 age and gender matched controls were recruited.
Participants were evaluated using The Yale-Brown Obsessive-Compulsive Scale (YBOCS), the Responsibility Attitude Scale (RAS), and the Metacognition Questionnaire-30 (MCQ-30).
Results: As hypothesized, the OCD group scored significantly higher than the control group in all dimensions in addition to the total score of the MCQ-30.
Effect sizes were highest for cognitive confidence, risks, control, and total score.
A strong positive correlation was found between dimensions of MCQ and YBOCS severity score.
After controlling for responsibility score, only two correlation coefficients for two subscales of MCQ-30 scale became non-significant.
Conclusions:Metacognition plays an important role in the pathogenesis of OCD as evidenced by elevated dysfunctional metacognitive beliefs in patients with OCD.
Cognitive therapies based upon cognitive formulations of OCD according to these results will be valuable in treating OCD symptoms.
