Background and objective : Atypical chest pain is a frequent chief complain of many young patients that consult their doctor in the hospital and daily private clinic , the atypical chest pain is the pain which doesn‟t match the criteria of ischemic cardiac chest pain , usually this pain described as pricking in nature .The objective of this study is to reveal the importance of echocardiography in the evaluation and diagnosis of young age patients with atypical chest pain , those age group which are routinely not investigated by this advance, simple readily available diagnostic tool.

Study design and method: 90 young patient with atypical chest pain were included , their age between 10-40 years, all of them had no history of any cardiac disease and compared with a 70 randomly selected age and sex matched healthy individuals control group , both group were examined by ECG , chest Xray and Echocardiography and cardiac enzyme and all of them have normal ECG and enzyme but the finding was abnormal in their echo examination .

Results: From those 90 young patient with atypical chest pain , 47 ( 52 %)of them have abnormal echo cardiography and from those 36 patients ( 45%) were revealed to have mitral valve prolapse , less for mitral stenosis and aortic stenosis ..

Conclusion : Echo cardiography should be a rotten examination for any young patient with non specific chest pain, and with specific concentration on mitral valve function and structure.
