In the later 20 years, the cardiac troponin testing has been available in clinical laboratories.

This biomarker has revolutionized testing of patients with acute coronary syndromes.

However, traditional troponin assays show some limits: the relatively long time elapsing between the onset of ischemia and the increase in serum concentration، and the difficulty in distinguishing ischemic from non ischemic damage.

Cardiac troponin I and T testing has become the cornerstone for diagnosis of myocardial infarction and is useful for risk assessment and management of suspected acute coronary syndrome patients.

Its introduction in clinical practice consistently improved both sensibility and specificity as compared with other biomarkers, such as creatin-kinase MB.

It has become clear that even small troponin elevations are associated with adverse health outcomes.

There have been several generations of troponin assays, all toward tests that reliably detect lower concentrations of this critical analyte.

These generations of cardiac troponin I and T assays will likely be termed 'high sensitivity' and can be useful in diagnosis of myocardial infarction earlier.

These assays should have the ability to measure troponin with a CV of total error of <10% at concentrations significantly lower than the 99 percentile of the normal reference population.
