Abstract EN
Objective : To study cases of iatrogenic cholesteatoma after myringoplasty to identify the factors involved and plan their treatment.
Materials and Methods: Study is a retrospective analysis of 11 patients (11 ears) with iatrogenic choléseatome after myringoplasty.
Results: There were 11 ears among 300 ears operated a myringoplasty an incidence of 3.6%.
The time to onset of symptoms compared to myringoplasty was 18 months on average (range, 3 months and 4 years).
This was the recurrence of otorrhea in all cases.
The examination showed a whitish opacity behind the neotympanum in 4 cases at the external auditory meatus in 4 cases.
CT spoke diagnosis in 3 cases.
Four patients were reoperated under local anesthesia ablation Pearl choléseatome in the external ear canal while seven patients underwent reoperation under general anesthesia.
Cholesteatoma was located at hypotympanique level in two cases and protympanique level in one case, while diffuse antralatticalmésotympanique and in 4 cases.
Conclusion : The iatrogenic cholesteatoma is a rare complication of myringoplasty with a post-operative monitoring is required for early diagnosis.

Objectif : Etudier le choléstéatome iatrogène après myringoplastie afin de dégager les facteurs en cause et planifier leur traitement.
Matériel et méthodes : Etude rétrospective portant sur 11 oreilles opérées d’un choléstéatome iatrogène après une myringoplastie entre 2002 et 2012.
Résultats : Il s’agissait de 11 oreilles atteintes d’un choléstéatome iatrogène parmi 300 oreilles opérées d’une myringoplastie soit une incidence de 3,6%.
Le délai d’apparition des symptômes par rapport à la myringoplastie était de 18 mois en moyenne (extrêmes de 3 mois et 4 ans).
Il s’agissait de récidive de l’otorrhée dans tous les cas.
L’examen avait montré une opacité blanchâtre derrière le néotympan dans 4 cas et au niveau du conduit auditif externe dans 4 cas.
La TDM a évoqué le diagnostic dans 3 cas.
Quatre patients ont été réopérés, sous anesthésie locale, d’une ablation de perle de choléstéatome au niveau du conduit auditif externe alors que sept patients ont été réopérés sous anesthésie générale.
Le choléstéatome a été localisé au niveau hypotympanique dans deux cas et au niveau protympanique dans un cas, alors qu’il était diffus antro-attical et mésotympanique dans 4 cas.
Conclusion : Le choléstéatome iatrogène est une complication rare de la myringoplastie.
Une surveillance post opératoire est nécessaire pour le diagnostiquer précocement.
