Background & Objective: This retrospective study is to correlate oral squamous cell carcinoma with age, gender and location of lesion, and to investigate the correlation between the grade of the malignancy and lymph nodes in patients of the faculty of dentistry, Damascus university.

Methods & Materials: Clinical files and biopsies of oral squamous cell carcinoma were checked in 300 patients aged 24 – 68 years, average 46, 180 males – 120 females, at department of oral histology and pathology between 2002 and 2009.

Analytical tables were designed using SPSS program, issue 13.0.

Chi square test at sig<0.05 was used to study the correlation between lesion, risk factors, age, gender, location of lesion on keratinized and non-keratinized mucosa, clinical form and the defeat of lymph nodes of the neck.

The study was conducted to investigate the degree of malignancy according to the World Health Organization (WHO).

Results: 82.7% of oral squamous cell carcinoma appeared after the age of 40.

60% of which defeated men, 63.4% of which in mandibular region, 68.3% appeared in the form of ulcers.

Inferior lips formed more than the quarter of lesions by 27.4%.

Inferior lip was more than that of superior by 4:1.

For keratinized mucosa it was 46% and 54% for non-keratinized mucosa.

In the histological study, 37% cases were of the first grade and 26% of the second grade.

Defeated lymph nodes formed 52%.

The statistical study showed a strong relation, sig<0.05, between defeat of lymph nodes and the degree of histological malignancy.

Conclusion: Oral squamous cell carcinoma appeared at older ages of men and placed in the lower jaw in the form of ulcer.

There is a correlation between the degree of histological malignancy and lymph lesions.

This means that we must make sure of the diagnosis by histological examination and attention should be paid to chronic ulcerated lesions.
