Background& Objective: The need to repair dentoalveolar atrophy and bone defects has resulted in developing techniques and sources of graft materials، and the use of autogenous bone grafts continues to represent the ‘gold standard’ in reconstructive surgery of the oral and maxillofacial region.

This study aims to assessment the complications of autogenous bone harvesting from retromolar region of mandible by trephine burs.

Materials & Methods: This study included 12 patients (3male، 9 female)، the patients ages ranged from 21 to 52 years (mean 40،4).

These patients have a lack in the width of the alveolar bone in the posterior regions of mandible.

This lack was regenerated by application of two stage alveolar bone-split technique.

The gaps resulted from splitting were filled by autogenous bone grafts obtained from retromolar region of mandible by trephine burs.

CBCT was taken for each patient: before، immediately after، and 4 months after the surgery.

Soft tissue superficial sensory function was assessed by the Pointed-Blunt Test and the Two-Point-Discrimination Test.

Evaluation of the pulp sensitivity of teeth was carried out by cold vitality test.

The clinical complications of surgical procedure were assessed during the surgery and healing phase.

Results: The results of the Pointed-Blunt Test and the Two-Point-Discrimination Test showed no alteration in the soft tissue superficial sensory function.

The results of the cold vitality test showed no alterations in the vitality of the teeth.

Also there were no clinical complications during surgery and healing period.

Conclusion: the results of clinical and radiographic study showed no complications in regions of harvesting grafts, and this suggest that using trephine burs for harvesting bone graft from retromolar region is save.
