Background: Three-dimensional seal of the root canal system is one of the main goals of endodontic treatment.

Aim: The purpose of this study was to evaluate the effect of smear-layer removal using Nd:YAG laser and MTAD on apical leakage of obturated root canals using a dye-leakage test.

Methods: A total of 135 extracted, single-rooted, human teeth were cleaned and shaped using pro-taper rotary instruments under irrigation with 5.25% sodium hypochlorite and divided into 3 equal groups of 45 each (groups A,B,C).

The smear layer in groups A and B was removed using pulsed Nd:YAG laser(Pulse Master 600.0, BIOLASE U.S.A) or MTAD (Dentsply Tulsa, Tulsa Ok), respectively.

The smear layer in group C was left intact (control).

Then each main group divided into 3 subgroups (n=15) according to the sealer used.

Teeth were obturated by lateral condensation with gutta-percha and either Z.O.B.Seal, or Apexit Pluse, or AH Plus and incubated for one week at 37°C.

The teeth were immersed in India ink for 5 days and then cleared to visualize the level of apical microleakage with stereomicroscope.

The results were evaluated by one-way ANOVA and Tukey’s test.

Results: The result showed statistically significant difference between MTAD group and control , as showed between MTAD group and laser group with the best result for MTAD (0.42 ± 0.62) regardless of the sealer used.

There was no significant difference between laser treated groups and the control group.

With regard to the root canal sealers, Apexit Plus showed significantly more sealing ability than Z.O.B.Seal in MTAD and control groups.

While in laser treated group there was no significant difference between sealers.

Conclusion: It was concluded that the use of MTAD as a final rinse improve the apical sealing in obturated root canals.
