Introduction : The functional ovarian cyst remains a rare disease in the prepubertal population; it is however a leading cause of precocious pseudo-puberty.
Precocious puberty results in significant changes marked by growth acceleration and premature closure of growth plates.
We report in this article the case of a precocious pseudopuberty secondary to a functional ovarian cyst.
Case presentation : A six and a half years old patient, consulted for a recent appearance of early breast development.
Hormonal analysis revealed a distal hormonal secretion with suppression of the hypothalamic-pituitary axis.
Pelvic ultrasound revealed the presence of a large right ovarian cyst, confirmed by pelvic MRI.
The management of this patient consisted in a simple monitoring.
After two months, a complete regression of the symptoms was noted as well as a complete disappearance of the ovarian cyst on ultrasound, and an elimination of the hypothalamic-pituitary axis suppression on hormonal analysis.
Conclusion : Ovarian cysts are mostly self-limited and require no treatment with spontaneous regression.
In rare cases surgery is necessary; it is important to consider a conservative intervention of ovarian tissue without affecting fertility.
Ovarian cysts are rare in the pediatric population and with good prognosis in their majority; they require however the attention of the pediatrician, given the possible risk of malignancy.

Le kyste fonctionnel de l’ovaire reste une maladie rare dans la population prépubère, il est cependant une des causes majeures de pseudo-puberté précoce.
La puberté précoce entraîne des modifications importantes marquées par l’accélération de la croissance et la fermeture prématurée des cartilages de croissance.
Nous rapportons ici un cas de pseudopuberté précoce secondaire à un kyste ovarien fonctionnel.
Présentation du cas : Patiente âgée de six ans et demi, consultant pour l’apparition récente de signes de puberté.
Le bilan hormonal révèle une sécrétion hormonale distale avec suppression de l’axe hypothalamo-hypophysaire.
L’échographie pelvienne révèle la présence d’un kyste de l’ovaire droit, confirmée par IRM pelvienne.
La prise en charge a consisté en une simple surveillance.
Après deux mois, une régression complète des symptômes a été notée, ainsi qu’une disparition totale du kyste ovarien à l’échographie et une élimination de la suppression de l’axe hypothalamo-hypophysaire sur le bilan hormonal.
Conclusion: Les kystes ovariens sont souvent auto-limités avec régression spontanée, et ne nécessitent aucun traitement.
Dans de rares cas, la chirurgie est nécessaire, et il est important d’envisager une intervention conservatrice du tissu ovarien sans affecter la fertilité.
Les kystes ovariens sont rares dans la population pédiatrique et de bon pronostic dans leur majorité, mais ils exigent l’attention du pédiatre, étant donné l’impact possible du retard diagnostique sur le pronostic, la croissance et la fertilité.
