High tibial osteotomy (HTO) is commonly used for genu varum deformity in young and active patients.
Corrective valgus osteotomy may however lead to an oblique joint line in cases of associated femur varum or absence of tibia vara.
To avoid this drawback, we use an accurate and reproducible radiological protocol allowing to choose the best indication between HTO, double level osteotomy (DLO) and distal femoral osteotomy (DFO).
Computer-navigation of the osteotomies is the best choice to achieve the preoperative goal, above all to avoid too much overcorrection, which could be detrimental to resume sports.
In this article, the rationale of this choice, the operative procedures, their results as well as the possibilities to resume sports will be presented

chez les patients jeunes et actifs pour traiter le genu varum.
Cependant, l’ostéotomie de correction peut conduire à un interligne oblique en cas de varus fémoral associé ou en l’absence de tibia vara.
Afin d’éviter cette situation, nous utilisons un protocole radiologique reproductible qui permet de choisir la meilleure indication entre ostéotomie tibiale de valgisation (OTV), double ostéotomie et ostéotomie fémorale distale.
La navigation informatisée des ostéotomies est le meilleur choix pour atteindre l’objectif préopératoire, et tout particulièrement, pour éviter une trop grande hypercorrection qui pourrait compromettre la reprise sportive.
Dans cet article, seront présentés les raisons de ce choix, les techniques opératoires et leurs résultats ainsi que les possibilités de reprise du sport.
