Boerhaave’s syndrome or spontaneous esophageal perforation secondary to pyloric stenosis is a very rare condition with an incidence of 0.5 per million per year.
We report the case of a 48-year-old gentleman presented with Boerhaave’s syndrome that required surgical intervention.
The patient was managed with a single-stage procedure in the form of a primary repair of the esophagus, buttressed with a pleural flap and a loop gastrojejunostomy.
A review of the literature found this to be the first case managed using a single-stage procedure.
This may be considered as a surgical option in the treatment of spontaneous esophageal perforation secondary to pyloric stenosis to avoid a two-stage operation and its consequences.

Le syndrome de Boerhaave ou perforation sponta-née de l’oesophage secondaire à une sténose du pylore est rarement vu, son incidence est de 0,5 pour 1 million par an.
Nous rapportons le cas d’un homme de 48 ans avec un syndrome de Boerhaave qui a nécessité une intervention chirurgicale.
Le patient a été traité par une intervention d’emblée comportant une réparation primaire de l’oesophage avec un lambeau pleural et une anastomose gastrojéjunale.
Selon la revue de la littérature, il s’agit du premier cas à être géré en une chirurgie à une seule étape.
Cette technique pourra être considérée comme une option chirurgicale dans le traitement de perforation spontanée de l’oesophage secondaire à une sténose du pylore, permettant ainsi d’éviter une intervention en deux étapes avec les séquelles qui en découlent
