Plasma fibronectin (FN) and urinary glycosaminoglycans (GAG) were studies in non complicated, retinopathy-complicated and nephropathy- complicated non insulin-dependent diabetic patients (NIDD).
Doth plasma FN and.
urinary glycosaminoglycans were significantly higher in all diabetic subgroups compared to control group.
No significant difference was observed between diabetic subgroups as regard plasma FN, whereas ncphropathic subgroup excrcte more GAG than the other diabetic groups.
Negative correlation was observed between urinary GAG and duration in non complicated diabetics.
Positive correlation was found between urinary GAG and 24 hours urine protein in retinopathy complicated subgroup.
Furthermore, positive correlations were found between FN and each of fasting and post-prandial plasma glucose level in control group.
Also, positive correlation was observed between plasma FN and urine GAG in non complicated diabetic patients.
Increased plasma FN is related to augmented production by vascular endothelium, platelets, and other mesenchymal cells.
Deposition of FN in vascular lesions and in vascular and glomerular basement membranes play an important role in the vascular complications of diabetes and alter the permeability of glomerular basement membrane.
In addition to FN, lysosomal enzymes might be implicated in enhanced degradation of proteoglycans and increased permeability of glomerular basement membrane with subsequent increase of urinary GAG.
The latter can be considered as an early marker of renal involvement in diabetes, before the onset of proteinuria.
