Abstract EN

Background and Objective: poorly understood , life threatening, and often unconsidered complication of blood components transfusion complicated by acute lung injury (ALI ), which may progress to (ARDS), Transfusion- related acute lung injury (TRALI)has been emerging in the last decades and needs more investigations , studies and documenting new cases.

Methods and Materials: we report a case of 40 years old man with respiratory failure after transfusion of fresh frozen plasma and blood during abdominal surgery, who developed hypoxia, dyspnea, tachycardia, hypotension and bilateral pulmonary infiltrations.

No specific diagnostic manifestations were found and a diagnosis of transfusion -related acute lung injury was made after excluding other possible causes.

Conclusion: TRALI needs more attention to diagnose and manage cases which may threat life and can be prevented at the same time .an important message is sent to blood bank to investigate implicated donors per local policy and report fatal cases and non fatal cases .local studies in anesthesia & surgical intensive care and hematology department and cooperation between intensivists and hematologists are wanted.
