Objective: to find out whether preeclampsia is associated with alterations in insulin–like growth factor-1 (IGF-1) and its binding proteins: insulinlike growth factor binding proteins 1 and 3 (IGFBP 1 and 3) independent from the changes attributed to reduced birth weight.

Patients & Methods: The study included 150 pregnant women in labor from Ain Shams University Maternal Hospital who were divided into 3 equal groups: Group (1) severe preeclampsia (PE), group (2) mild PE and group (3) normotensive (control) groups.

Cord blood was collected from each participant for measurement of plasma IGF-1, IGFBP1 and IGFBP-3.

Neonatal weight was recorded after delivery and fetal growth was expressed as the ratio between the observed and expected birth weights.

-Results: There were no differences between mild PE and controls regarding IGF-1, IGFBP-1 and IGFBP-3.

In severe and early onset PE, umbilical cord plasma IGF-1 was significantly lower (P < 0.01) and IGFBP-1 was The Relation of Insulin Nehad Shoeib, et al significantly higher (P <0.01) than in controls.

For each birth weight, IGF-1 was lower and IGFBP-1 was higher in severe or early PE than among controls of similar weight.

Birth weight and PE were, independent of each other, associated with IGF-1 whereas birth weight but not PE, was associated with IGFBP-1, after adjustment of gestational age. Conclusion: Fetal growth restriction caused by severe or early PE is associated with lower umbilical levels of IGF-1 than low birth weight caused by other conditions.

PE may contribute to the observed IGF-1 reduction either as part of the underlying causes of PE or as a consequence of the disease
