Dilated cardiomyopathy is the most common cardiomyopathy in childhood.
It’s defined by the presence of left ventricular dilatation with systolic dysfunction.
The clinical presentation usually involves cardiac failure, which is correlated to the degree of myocardial dysfunction.
Both echocardiography and cardiac magnetic resonance are of great value in the diagnosis and assessment of cardiac function in pediatric patients with dilated cardiomyopathy.
The Dilated cardiomyopathy of the child is classified in primary and secondary Dilated cardiomyopathy.
Primary dilated cardiomyopathy has hereditary or metabolic origin, while secondary dilated cardiomyopathy may be the consequ ence of a left obstacle, an old left-right shunt, a disorder of rhythm or conduction, a birth anomaly of coronary arteries, or viral or autoimmune myocarditis.
The management is based mainly on medical therapy for congestive heart failure, immune modulating therapy in case of myocarditis and cardiac transplantation.

La cardiomyopathie dilatée (CMD) est la cardiomyopathie la plus fréquente de l'enfant.
Elle est définie par la présence d'une dilatation ventriculaire gauche avec une dysfonction systolique.
La présentation clinique est généralement celle d’une insuffisance cardiaque qui corrélée au degré de la dysfonction myocardique.
L'échocardiographie cardiaque et l’imagerie par résonance magnétique sont très utiles pour le diagnostic et l'évaluation de la fonction cardiaque.
Les CMD de l’enfant sont classés en CMD primitives et secondaires.
Les CMD primitives sont d’origine héréditaire ou métabolique alors que les CMD secondaires peuvent être la conséquence d’un obstacle gauche, d’un shunt gauche-droite de longue évolution, d’un trouble du rythme ou de la conduction, d’une anomalie de naissance des artères coronaires, ou d’une myocardite virale ou auto-immune.
La prise en charge repose sur le traitement médical de l'insuffisance cardiaque congestive, le traitement immuno-modulateur en cas de myocardite et la transplantation cardiaque.
