Introduction : sclerosing cholangitis is an inflammatory and fibrosing lesion of intra or extra hepatic bile ducts.
this condition can be primary or secondary when an etiology can be defined.
we report a case of primary sclerosing cholangitis revealed by acute a hepatitis.
case report: a twelve-year-old girl presented with recurrent abdominal pain.
primary investigations revealed cholestasis and the diagnosis of acute hepatitis a.
however, the persistence of cholestasis has led to performing an mr-cholangiography that showed multiple segmental strictures and irregularity of bile ducts’ wall suggesting sclerosing cholangitis.
etiological investigations concluded to a primary sclerosing cholangitis conclusion : in front of uncommon evolution of acute hepatitis a, further investigations must be performed to search for signs of chronic hepatopathy that may be revealed by intercurrent infection.

Introduction : la cholangite sclérosante est caractérisée par une atteinte inflammatoire et fibrosante des voies biliaires intra et/ou extra hépatiques.
nous rapportons l’observation d’une cholangite sclérosante primitive révélée par une hépatite a.
observation : il s’agit d’une fille âgée de 12 ans sans antécédents pathologiques particuliers admise pour l’exploration d’une hépatomégalie sensible.
le diagnostic d’hépatite aigue a a été porté.
cependant, la persistance d’une cholestase a conduit à la réalisation d’une bili-irm.
cette exploration a révélé un aspect irrégulier des parois des canaux biliaires avec des images de sténoses multiples donnant un aspect en chapelet en faveur d’une cholangite sclérosante.
les investigations étiologiques ont conclu à une cholangite scérosante primitive.
conclusion : tout cas d’hépatite a d’évolution inhabituelle doit faire pousser les investigations à la recherche d’une hépatopathie chronique, quiescente jusque là, et révélée par l’infection intercurrente par le virus de l’hépatite A.
