Abstract EN
Introduction : nocardia infections are rare.
they are underestimated and misunderstood.
they occur mainly in case of immunosuppression.
observation : we report the case of a systemic nocardiosis occurred in a woman with an acquired human immunodeficiency syndrome.
the chest radiograph found a left lower lobe nodule.
brain and facial scan revealed a right frontal abscess, a frontal osteitis, a collection of the right temporal muscle and a collection of the upper eyelid and intra orbital.
the diagnosis of nocardiosis was retained since nocardia isolation after samples culture from different sites.
clinical and radiological outcome was favorable after stereotactic drainage of the brain abscess and adapted antibiotherapy based on ampicillin and cotrimoxazole.
conclusion : through this observation, we emphasize the value of early diagnosis of nocardiosis in immunocompromised patients specially to improve the prognosis generally redoubted in this field.

Introduction : les infections à nocardia sont rares.
elles restent sous estimées et méconnues.
elles surviennent surtout sur terrain d'immunodépression.
observation: nous rapportons l’observation d’une nocardiose systémique chez une patiente atteinte d’un syndrome d’immunodéficience humaine acquise.
la radiographie thoracique trouvait un nodule du lobe inférieur gauche.
le scanner cérébral et du massif facial révélait un abcès frontal droit, une ostéite frontale, une collection du muscle temporal droit et une collection de la paupière supérieure et en intra orbitaire.
le diagnostic de nocardiose était retenu devant l’isolement de nocardia à la culture de prélèvements des différents sites.
l'évolution clinique et radiologique était favorable après drainage stéréotaxique de l’abcès cérébral et une antibiothérapie prolongée à base d’ampicilline et de cotrimoxazole.
conclusion: a travers cette observation, nous soulignons l’intérêt de faire un diagnostic précoce de la nocardiose particulièrement chez l’immunodéprimé afin d’améliorer son pronostic, généralement redoutable sur ce terrain.
