Nephrotic syndrome is the most common glomerular disease in childhood.
Steroids are the first-line therapy in its management.
It allows a remission in 90% of cases.
However, the majority of children relapse.
So, they are exposed to acute complications (such as thromboembolic and infectious).
Also, chronic proteinuria is responsible for renal damage with interstitial lesions.
In the other hand, patients are exposed to the risks of toxicity of long-term corticosteroid therapy.
Therefore, steroid-sparing agents may be indicated in the case of steroid-dependency or frequent relapses.
However, no consensus exists regarding the different treatment options.
Abstract FRE
Le syndrome néphrotique idiopathique (SNI) est la néphropathie glomérulaire la plus fréquente de l’enfant.
La corticothérapie représente la pierre angulaire de sa prise en charge.
Elle permet l’obtention d’une rémission dans 90% des cas.
Cependant, la majorité des enfants rechutent.
A chaque poussée, ces patients sont exposés aux complications aigues (thromboemboliques et infectieuses) et chroniques du SNI puisque la protéinurie persistante entrainerait à long terme des lésions interstitielles.
Ils sont également exposés aux risques de toxicité de la corticothérapie au long cours.
Ainsi, des traitements d’épargne cortisonique doivent être utilisés en cas de SN corticodépendant de l’enfant.
Plusieurs molécules sont disponibles, mais il n’existe actuellement pas de consensus international.
De plus aucune alternative thérapeutique ne garantit une guérison définitive pour tous les malades.
