English Abstract

Background: Diabetic nephropathy (DN) is one of the most common and a major lifethreatening microvascular complication of diabetes mellitus (DM), accounting for 20 - 40% of populations with Type 2 diabetes mellitus (T2DM) and may lead to end stage renal disease (ESRD), thus for survival require either kidney transplantation or dialysis.

Neutrophil gelatinase-associated lipocalin (NGAL) is a glycoprotein (25- kDa) which consists of a polypeptide chain of 178 amino acids and belongs to the ‘lipocalin’ superfamily.

Serum and urinary NGAL can be identified starting 2-4 hours after acute kidney injury (AKI).

Some recent studies demonstrated that NGAL may be considered as an early biomarker for AKI.

Objective: To assess the role of urinary NGAL for early detection of DN among a group of T2DM patients in Gaza.

Materials and methods: This case control study comprised 75 persons (50 cases “25 T2DM patients with normoalbuminuria (group I) and 25 T2DM with microalbuminuria (group II)” & 25 healthy controls).

Questionnaire interviews were applied to the study population.

Urine NGAL level, fasting blood glucose (FBG), serum creatinine, serum urea, HbA1c%, total cholesterol, triglycerides, high density lipoprotein (HDL-c) and low-density lipoprotein (LDL-c) and microalbumin were determined.

Blood pressure was also measured.

Body mass index (BMI) and Glomerular filtration rate (GFR) were calculated.

Approval was acquired from Ethical Research Committee, Islamic University of Gaza to perform this study.

All data were analyzed by a computer using SPSS program.

Results: The mean level of urine NGAL and microalbumin in Group II were significantly higher than that in Group I and controls (132.5 ± 117.2 ng/ml & 89.1 ± 59.7 mg/g versus 26.5 ± 18.5, 27.2 ± 17.7 ng/ml and 22.6 ± 4.8, 16.4 ± 5 mg/g respectively with p < 0.001), but there was no significant difference between normoalbuminuria cases and control subjects.

The Pearson correlation test showed positive significant correlations between urine NGAL levels and SBP, DBP, FBG, HbA1c% and microalbumin (r = 0.260, p = 0.024; r = 0.257, p = 0.026; r = 0.227, p = 0.049; r = 0.310, p = 0.007 and r = 0.555, p < 0.001 respectively).

On the other hand, negative significant correlation was showed with serum creatinine (r = -0.273, p = 0.018).

Regression analysis showed that microalbumin, HbA1c%, serum creatinine, SBP, DBP and glucose were the independent predictors of urinary NGAL.

Conclusions: Urine NGAL as a tubular marker may be early elevated in type 2 diabetic patients and may be considered as a noninvasive predictive marker for early detection of DN in patients with T2DM.
