Introduction: Diaphysar bone segments reconstruction is always a major challenge regardless of the etiology of bone loss.
Beyond 6 cm, autologous bone grafts undergo a resorption phenomenon even in a well-vascularized muscle environment.
Observation: A 46-year-old woman operated on in 2002 for a fracture of two bones in her right forearm complicated by a sepsis on equipment.
She ended up with a wide resection that left a bone loss of 15cm.
Ten years later, faced with the presence of an osteoma of the forearm and productive fistulas, she consulted us to call for the amputation of the forearm.
We did a reconstruction of the forearm using the induced membrane technique using an un-vascularized fibula graft.
The evolution was marked by the incorporation of the graft with acceptable result.
Conclusion: The use of fibula with the technique of the induced membrane gives more chance for consolidation

Introduction : La reconstruction osseuse diaphysaire représente un défi majeur quelque soit l’étiologie de la perte de substance.
Au-delà de 6 cm, les greffes autologues subissent un phénomène de résorption même dans un environnement musculaire bien vascularisé.
Observation : Une femme âgée de 46 ans a été opérée pour une fracture des deux os de l’avant-bras droit compliquée d’un sepsis sur matériel.
Une résection large qui a laissé une perte de substance osseuse de 15 cm a été réalisée.
Dix ans après, devant la présence d’un ostéome de l’avant-bras et de fistules productives, elle a consulté pour réclamer l’amputation de l’avant-bras.
Nous avons fait une reconstruction de l’avantbras selon la technique de la membrane induite en utilisant un greffon de fibula non vascularisé.
L’évolution a été marquée par l’incorporation du greffon avec un résultat acceptable Conclusion : L’utilisation du fibula avec la technique de la membrane induite donne plus de chance pour la consolidation
