Pituitary hyperplasia due to primary hypothyroidism is a rare and underdiagnosed disease.
Its main differential diagnosis is pituitary adenoma.
We report three cases of pituitary hyperplasia, the first about an 18-year-old patient who presented with stature delay, the second case involved a 27-year-old woman for whom the discovery was incidental during a screening for her retro-orbital neuropathy, and in the latter case it was a 31-year-old patient who initially consulted for intracranial hypertension.
In all cases, imaging showed a pituitary macroadenoma and plasma thyrotropin-stimulating hormone level was very high (greater than 100 mIU / mL).
Hormone replacement therapy resulted in clinical and radiological improvement with regression of pituitary masses.
Pituitary hyperplasia secondary to primary hypothyroidism requires multidisciplinary management and must be considered for patients with pituitary and suprasellar masses in order to avoid unnecessary surgery

L’hyperplasie hypophysaire est définie par une augmentation de la taille de l’hypophyse suite à une prolifération des cellules thyréotropes secondairement à une hypothyroïdie périphérique prolongée.
l’objectif de ce travail est de différencier entre adénome et hyperplasie hypophysaire afin d’éviter une conduite abusive.
le premier cas rapporté est à propos d’un patient âgé de 18 ans qui avait un retard staturopondéral, le deuxième concernait une patiente âgée de 27 ans chez qui la découverte de l’hyperplasie était fortuite, et dans le dernier cas il s’agissait d’une patiente âgée de 31 ans qui a consulté initialement pour un syndrome d’hypertension intracrânienne.
dans tous les cas, l’imagerie avait montré un aspect en faveur d’un macro adénome hypophysaire et à la biologie le taux de thyréostimuline était supérieur à 100 mui/ml.
l’hormonothérapie substitutive avait entraîné une régression de l’hyperplasie hypophysaire.
l’hyperplasie hypophysaire secondaire à l’hypothyroïdie périphérique reste rare et sous diagnostiquée et nécessite une conduite thérapeutique particulière.
