Mal de pott cervical : cause rare d'une tumefaction

Pott's disease is exceptional.

It's one of the rare causes of cervical suppuration.

We report a case of a forty seven years old woman with a left cervical swelling, torticollis, evolving for 3-month, night sweats and difficulty in swallowing.

A cervical Computed Tomography showed atlanto-axial lysis with retropharyngeal abscess which fuse to the latero-cervical region.

An adenectomy of right lymphadenopathy under local anesthesia was done and the result of the pathology examination was in favor of lymph node tuberculosis.

An MRI was requested, which confirmed the presence of a large abscess on the left lateral posterior wall with lateral exteriorization in the neck.

Antituberculosis therapy was initiated and drainage of the abscess was performed.

The duration of antibiotic therapy was 13 months with cervical immobilization.

Results at two years months' follow-up were satisfactory.

Conclusion : The cervical abscess complicating a cervical Pott disease is exceptional.

It needs to be considered in endemic regions.

Imaging is highly contributive to diagnosis and to follow-up of lesions in regression under treatment.

Le mal de Pott cervical est une localisation très rare de la tuberculose osseuse.

Nous rapportons l'observation d'une femme de 47 ans présentant une tuméfaction latéro-cervicale gauche avec une adénopathie latéro-cervicale droite basse associée à un torticolis, évoluant depuis 3 mois, des sueurs nocturnes et une altération de l'état général.

Une TDM cervicale a montré une lyse atlanto-axiale avec un abcès rétro-pharyngé qui fuse vers la région latéro-cervicale.

Une adénectomie sous anesthésie locale a été faite et le résultat de l'examen anatomo-pathologique était en faveur d'une tuberculose ganglionnaire.

L'IRM a confirmé la présence d'un abcès froid latéro-cervical gauche avec extension à la nuque.

La patiente a bénéficié d'un drainage de l'abcès, d'une immobilisation cervicale et de 13 mois de traitement antituberculeux.

L'évolution était favorable avec un recul de deux ans.

Conclusion : L'abcès cervical froid compliquant un mal de pott cervical est exceptionnel.

Cette forme particulière doit toujours être suspectée dans les zones endémiques.

L'imagerie contribue au diagnostic et permet le suivi des patients.
