الصمم ينتج عن تأثر السمع الدفاعات المضادة للأكسدة، مما يؤدي إلى زيادة في كمية ROS.
هذه النتائج تشير إلى وجود علاقة سببيه بين ROS، و الإجهاد الوظيفي لمضادات الأكسدة و الضرر الذي يصيب الأذن بالصمم.
الهدف : لإلقاء مزيد من الضوء على العلاقة بين عنصر الإجهاد الوظيفي لمضادات الأكسدة و فقد السمع الوراثي.
و قد قمنا في هذه الدراسة بالكشف الإكلينيكي الدقيق و معرفة التاريخ المرضي للمرضى و إذا كان هناك حالات مشابهة في الأسرة أو زواج للأقارب في المرضى المترددين على عيادة الوراثة الإكلينيكية بالمركز القومي للبحوث و أيضا تم قياس السمع لهم إلى جانب قياس تركيزات البلازما من النحاس و الزنك و الكالسيوم، و الفوسفور، و الماغنسيوم و الحديد.
و فضلا عن ذلك، lipid peroxidation و B كاروتين، و فيتامين أ و فيتامين ج و فيتامين هـ إلى جانب النشاط المناعي للجسم.
النتائج : انخفاض ملحوظ في الكالسيوم، و الفوسفور، B كاروتين، و فيتامين هـ فضلا عن تدني مستويات الجهاز المناعي و زيادة في lipid peroxidation مما يدل على وجود الإجهاد الوظيفي لمضادات الأكسدة و يشير إلى وجود دور لمضادات الأكسدة في الآلية المرضية للصمم.
الخلاصة : نتائج البحث تؤيد فرضية أن النظام الغذائي و العوامل التي تؤثر على المناعة تؤثر على السمع و تؤدي إلى الصمم.
هناك حاجة إلى مزيد من الدراسات للتحقق مما إذا كانت مضادات الأكسدة و تصحيح قصور المغذيات من شأنه أن يحسن السمع في المرضى.
Abstract EN
Objectives: Ototoxicity seems to result from the inhibition of cochlear antioxidant defenses, causing an increase in the amount of reactive oxygen species.
These findings suggest a causal relationship between the formation of reactive oxygen species, oxidative stress and functional/morphological ear damage. Methods: auditory function was monitored along with plasma concentrations of copper, zinc, calcium, phosphorus, magnesium and iron.
Further more, lipid peroxidation, ? carotene, vitamin A, E and C activities and immunoglobulin G, M and A levels were estimated. Results: A significant decrease in calcium, phosphorus, ? carotene, and vitamin E activities as well as low levels of immunoglobulin G and M were found.
The observed increase in lipid peroxidation is indicative for oxidative stress which suggests a putative role of antioxidants in the pathogenesis of sensorineural hearing loss. Conclusion: The results support the hypothesis that dietary and immune factors influence individual susceptibility to hearing loss. Further studies are needed to verify whether antioxidants, correction of deficient nutrients and/or immune modulation would improve sensorineural hearing loss.
