Parathyroid hormone and bone minerals in Sudanese patients with end-stage renal disease : pre and post hemodialysis

English Abstract

-Background: End-stage renal disease (ESRD) is associated with changes in bone minerals and parathyroid hormone (PTH). Objective: The study aimed to determine serum levels of parathyroid hormone, calcium and phosphate in patients with ESRD and its relation to hemodialysis. Methods: A cross sectional case-control study during the period of study at three hemodialysis units in Omdurman.

One hundred patients with ESRD on regular hemodialysis participated in the study.

Fifty healthy subjects acted as controls.

History and physical findings were documented and blood samples collected for analysis of PTH, bone minerals and other tests. Results: Out of 100 patients, 69 were males and 31 were females.

Age ranged from 15 to 80 years (mean was 47.5 years ± 12.5).

The study found high serum levels of PTH in the patients before dialysis (119.7 pg/ml) compared to controls (31.0 pg/ml), and significant decreased of serum PTH levels after dialysis (61.3 pg/ml).

Serum calcium concentration was low in the patients before dialysis compared to controls (6.8 mg/dl vs.

9.3 mg/dl) and it increased significantly after hemodialysis (7.8 mg/dl).

In contrast the serum phosphate concentration before dialysis was high in patients compared to controls (5.45mg/dl vs.

3.59 mg/dl).

However, it dropped to 3.8 mg/dl after hemodialysis.

The study found no significant correlation between PTH levels and age, cause of ESRD, duration of disease, and frequency of hemodialysis sessions.

However, there was gender difference on PTH levels being lower in males compared to that in females
