Study of growth hormone and insulin- likegrowth factor-1 in type 1 diabetes mellituswith diabetic nephropathy

BACKGROUND : Patients with type 1 diabetes exhibit abnormalities of the growth hormone (GH), Insulin-like growth factor (IGF) and Insulin-like growth factor binding protein-3 (IGFBP-3) axis, including GH hpyersecretion, reduced circulating levels of IGF-I and IGFBP-3.

These changes may contribute to the pathogenesis of diabetic-specific complications, including diabetic neuropathy, neophropathy and retinopathy.

Diabetic nephropathy is one of the most common secondary complications of diabetes mellitus.

GH and IGF-I have suggested to be involved in diabetic nephropathy, at least in the early renal hypertrophy/hyper function period.

• OBJECTIVE : The present work aimed to study the possible relationship of these hormonal factors (GH, IGF-I and IGFBP-3) to diabetic nephropathy.

• STUDY Design : We conducted our study on 50 subjects, 30 type I diabetic patients and 20 healthy persons.

The 30 diabetic patients were 27 patients with complications [22 with diabetic nephropathy, (12 patients had diabetic nephropathy with normal albuminuria, 9 patients had microalbuminuria and 1 patient had diabetic nephropathy with macroalbuminuria) and 5 complicated diabetic patients wihtout nephropathy] and 3 diabetic patients had no complications.

Our patients age ranged from 15 to 35 years.

Patients with secondary form of diabetes, chronic liver diasease, chronic kidney diasease, hypertension and short stature were excluded.
