Acute transverse myelitis, an acute inflammation of the spinal cord, can be caused by HSV infection.
We report a case of an 85-year-old man who presented with lower extremity weakness and flare-up of a vesicular rash on the sacral area.
The patient was diagnosed with acute transverse myelitis based on MRI and CSF findings.
CSF analysis showed a lymphocytic pleocytosis and positive HSV-2 PCR.
The patient was started on intravenous acyclovir, with full clinical and radiologic improvement despite persistence of HSV-2 PCR positivity in the CSF for four months.
A review of the literature of HSV myelitis is included.
We conclude that testing of HSV PCR on repeated CSF samples might not accurately reflect viral activity in the setting of clinical improvement

La myélite transverse est une inflammation aiguë de la moelle épinière parfois secondaire à une infection par HSV.
Nous rapportons le cas d’un homme de 85 ans présentant une faiblesse progressive des membres inférieurs accompagnée deux semaines auparavant d’une éruption cutanée dans la région sacrée.
Ponction lombaire et IRM étaient en faveur d’une myélite transversale aigüe.
L’analyse du LCR révélait une pléiocytose à prédominance lymphocytaire.
La sérologie de l’herpès simplex 2 était initialement négative dans le LCR (IgM et IgG).
Deux jours plus tard le PCR du HSV2 se révèlera positif.
Le patient a été mis sous acyclovir par voie intraveineuse.
Ce cas est particulier: le patient s’est complètement rétabli (sur les plans clinique et radiologique) malgré la positivité continuelle du PCR HSV dans le LCR 4 mois plus tard.
Sur ce point, nous proposons 3 hypothèses : • détection de particules virales mortes par le PCR au lieu du virus vivant • persistance de l’excrétion du virus HSV avec invasion directe de la moelle épinière plutôt que de causer des dommages par voie immunologique • et une possible contamination
